프로포폴

3. 작용 기전

프로포폴은 여러 작용 기전을 갖는 것으로 제안되었으며,^[75][76][77] GABA_A 수용체 활성을 증폭시켜 GABA_A 수용체 양성 알로스테릭 조절제로 작용하여 채널 닫힘 시간을 늦춘다. 고용량에서는 프로포폴이 GABA가 없는 상태에서도 GABA_A 수용체를 활성화하여 GABA_A 수용체 효능제처럼 작용할 수 있다.^[78][79][80] 프로포폴 유사체는 또한 나트륨 채널 차단제로 작용하는 것으로 나타났다.^[81][82] 일부 연구에서는 내인성 칸나비노이드 시스템이 프로포폴의 마취 작용과 고유한 특성에 크게 기여할 수 있음을 시사하는데, 내인성 칸나비노이드 또한 수면, 통증 처리 및 구토의 생리적 조절에 중요한 역할을 하기 때문이다.^[83][86] 프로포폴로 전신 마취를 받은 환자를 대상으로 한 뇌파검사 연구에 따르면 프로포폴은 뇌의 정보 통합 능력을 현저하게 감소시킨다.^[84]

프로포폴은 효소 지방산 아미드 가수분해 효소의 억제제로서, 이 효소는 내인성 칸나비노이드 아난다미드 (AEA)를 대사한다. 프로포폴에 의한 내인성 칸나비노이드 시스템의 활성화는 AEA 이화 작용의 억제를 통해 전뇌의 AEA 함량을 유의하게 증가시키며, 이는 CB1 수용체 활성화를 통해 프로포폴의 진정 작용에 기여한다.^[85] 이는 프로포폴의 정신착란 및 항구토 특성을 설명할 수 있다. 반면, 흡입 마취제 투여 후 수술 후 메스꺼움과 구토의 발생률이 높으며, 이는 유도 후 최대 40분 동안 지속될 수 있는 전뇌 AEA 함량의 유의미한 감소에 기여한다.^[86]

4. 약동학

프로포폴은 체내에서 강하게 단백질과 결합하며, 간에서 글루쿠론산 포합을 통해 대사된다.^[163] 프로포폴의 소실 반감기는 2~24시간으로 추정되지만, 임상 효과의 지속 시간은 이보다 훨씬 짧다. 중추신경계에서 빠르게 다른 조직으로 재분포되어 대사되기 때문이다.^[164][165] 정맥 내 진정에 사용되는 경우, 프로포폴의 단회 투여에 의한 효과는 수분 이내에 사라지는 경우가 많다. 발현은 매우 빠르며, 15~30초 만에 효과가 나타난다.^[101] 프로포폴의 용도는 폭넓으며, 단시간 또는 장시간 진정, 더 나아가 전신 마취에도 사용될 수 있다. 프로포폴의 사용은 오피오이드 약물과 관련된 경우가 많은 구토를 동반하지 않는다는 특징이 있다. 이러한 신속한 발현과 회복의 특징, 그리고 기억 상실 효과^[166] 로 인해 진정 및 마취에 널리 사용되고 있다.

5. 부작용

프로포폴은 소량만 주입해도 가벼운 수면 상태를 유발하며, 투여량이 증가하면 호흡이 억제되거나 중지될 수 있다. 적절한 인공호흡 없이는 사망에 이를 수 있으며, 이러한 호흡 억제는 다른 수면 유도제나 마약성 진통제와 함께 사용하면 더욱 심해진다. 프로포폴은 기억 상실과 무의식 상태를 유발하지만, 통증 감소 효과는 없다. 따라서 전신 마취 시에는 펜타닐이나 레미펜타닐과 같은 합성 마약을 함께 투여한다.^[13] 프로포폴 투여 시 일부 환자는 황홀감이나 행복감을 느껴 약물 의존성이 생기기 쉽다. 가수 마이클 잭슨은 불면증 치료를 위해 프로포폴을 투여받다가 과량 투여로 인한 호흡 정지로 사망했다.

프로포폴은 의식 소실 및 진정 작용과 함께 생체 징후에도 영향을 미친다. 동맥과 정맥 확장을 통해 혈압을 낮추고, 심장 수축력과 심박수도 감소시킨다. 따라서 프로포폴 투여 시 심박수와 혈압이 동시에 저하되는 현상이 흔하게 나타나며, 노인이나 중환자의 경우 더욱 주의해야 한다. 프로포폴은 정맥 주사 시 통증을 유발할 수 있으며, 특히 작은 혈관에 주사할 때 더 심하다. 이러한 통증을 줄이기 위해 국소 마취제인 리도카인을 섞어 투여하기도 한다.^[56] 프로포폴은 불규칙한 근육 움직임을 유발할 수 있지만, 임상적으로 큰 의미는 없다. 악성 고열증을 유발하지 않으며, 포르피리아증 환자에게도 안전하게 사용할 수 있다.

프로포폴을 장기간 과량 투여하면 프로포폴 주입 증후군(PIS)을 유발할 수 있으며, 이는 대사성 산증을 일으킬 수 있다. 소아 중환자에서 PIS로 인한 사망 사례가 보고되었으며, 에피네프린 등의 카테콜아민이나 스테로이드를 함께 사용하는 중환자에게서 더 자주 발생한다.

프로포폴은 물에 잘 녹지 않아 콩기름이나 달걀 레시틴 성분을 이용해 수용액에 녹여 사용한다. 이러한 성분 때문에 정맥 주사 시 통증이 심했지만, 최근에는 수용액에 녹이는 성분을 개선한 리푸로 제제 등을 통해 통증을 줄이려는 노력이 이루어지고 있다. 리푸로 프로포폴액은 단독으로 사용해도 리도카인을 섞어 주입하는 것과 비슷한 효과를 가진다. 달걀 알레르기는 대부분 알부민이라는 단백질 성분 때문에 발생하므로, 프로포폴에 사용되는 레시틴 성분에는 알레르기 반응을 보이지 않는다.

프로포폴의 가장 흔한 부작용 중 하나는 주사 시 통증이며, 특히 작은 정맥에서 흔하게 나타난다. 이 통증은 감각 신경에서 발견되는 TRPA1^[55] 통증 수용체의 활성화로 인해 발생하며, 리도카인으로 사전 처치하면 완화될 수 있다.^[56] 큰 정맥(주관)에서 더 느린 속도로 주입할 경우 통증이 덜 발생한다. 환자는 프로포폴에 대한 반응에서 상당한 변동성을 보이며, 때로는 소량의 투여량으로도 심각한 진정 효과를 나타낸다.

추가적인 부작용으로는 저혈압 (혈압 강하)이 혈관 확장과 관련되어 나타나며, 유도 용량 투여 후 일시적인 무호흡과 뇌혈관 효과 등이 있다. 프로포폴은 많은 정맥 마취제에 비해 더 뚜렷한 혈역학적 효과를 나타낸다.^[130] 혈압이 30% 이상 하락하는 사례는 적어도 부분적으로 교감 신경 활동의 억제 때문인 것으로 생각된다.^[131] 이러한 효과는 프로포폴 투여량 및 투여 속도와 관련이 있다. 또한 아편 계열 진통제에 의해 강화될 수 있다.^[132]

프로포폴은 또한 직접적인 혈관 확장을 통해 전신 혈관 저항 감소, 심근 혈류 감소, 산소 소비 감소를 유발할 수 있다.^[133] 또한 소변이 녹색으로 변색될 수 있다는 보고도 있다.^[134]

프로포폴은 성인 중환자실 환경에서 널리 사용되지만, 약물과 관련된 부작용은 소아에서 더욱 우려되는 것으로 보인다. 1990년대에 프로포폴 진정과 관련된 중환자실에서 여러 어린이 사망 사례가 보고되면서 미국 식품의약국(FDA)은 경고를 발령했다.^[135]

호흡 억제제로서 프로포폴은 빈번하게 무호흡을 유발한다. 무호흡의 지속 시간은 사전 투약, 투여 용량 및 투여 속도와 같은 요인에 따라 달라질 수 있으며, 때로는 60초 이상 지속될 수 있다.^[136] 중추 호흡 구동의 억제 결과로, 프로포폴은 호흡수, 분당 환기량, 일회 호흡량, 평균 흡기 유량 및 기능 잔류 용량의 상당한 감소를 유발할 수 있다.^[130]

프로포폴 투여는 또한 뇌 혈류 감소, 뇌 대사 산소 소비 감소 및 두개 내압 감소를 초래한다.^[137] 또한, 프로포폴은 정상 안압을 가진 환자에서 안압을 최대 50%까지 감소시킬 수 있다.^[138]

더 심각하지만 드문 부작용으로는 근육긴장 이상이 있다.^[139] 다른 정맥 마취제와 마찬가지로 가벼운 근간대성 운동은 흔하게 나타난다. 프로포폴은 포르피린증 사용에 안전한 것으로 보이며, 악성 고열을 유발하는 것으로 알려져 있지 않다.

프로포폴은 또한 일부 개인에게 음경 지속 발기증을 유발한다고 보고되었으며,^[143][144] 일부 환자에서 렘수면을 억제하고 수면의 질을 악화시키는 것으로 관찰되었다.^[145]

드문 부작용에는 다음이 포함된다:^[146]

• 불안
• 시력 변화
• 흐린 소변
• 객혈
• 헛소리 또는 환각
• 배뇨 곤란
• 삼키기 어려움
• 안구 건조, 구강 건조, 코 건조 또는 인후 건조

6. 상호작용

프로포폴은 벤조디아제핀과 같은 호흡 억제제와 함께 투여될 경우 호흡 억제 효과가 증강된다.^[74][150]

7. 역사

프로포폴은 상업용 멸균 유제 제형으로 제조가 어렵다고 여겨진다.^[48][49][50] 처음에는 크레모포르를 사용하여 인체용으로 제형화되었지만, 이 초기 제형은 용납할 수 없을 정도로 많은 아나필락시스 사건과 관련이 있었다. 결국 1% 유제 형태로 대두유에 제조되었다.^[51] 멸균 유제는 복잡한 제형으로, 그 안정성은 미셀 크기 및 분포와 같은 많은 요인의 상호 작용에 달려 있다.^{[52] [53][54]}

존 B. 글렌은 임페리얼 케미컬 인더스트리 (ICI)의 수의사이자 연구원으로, 13년 동안 프로포폴을 개발했으며, 이 노력을 인정받아 2018년 래스커상 임상 연구 부문을 수상했다.

원래 ICI 35868로 개발된 프로포폴은 일련의 ''ortho''-알킬화 페놀의 마취 효능 및 약동학적 프로파일에 대한 광범위한 평가 및 구조-활성 관계 연구를 거쳐 선택되었다.^[89]

1973년 약물 후보로 처음 확인된 프로포폴은 크레모포 EL에 용해된 형태로 1977년 임상 시험에 들어갔다.^[90] 그러나 크레모포에 대한 아나필락시스 반응으로 인해 이 제형은 시장에서 철수되었고, 이후 대두유와 프로포폴 혼합물의 에멀젼 형태로 재구성되었다. 이 유화 제형은 1986년 ICI(제약 부문은 나중에 아스트라제네카의 구성 요소가 됨)에 의해 Diprivan이라는 브랜드 이름으로 다시 출시되었다.

이 제제는 프로포폴 1%, 대두유 10%, 유화제인 정제된 난황 인지질 1.2%, 글리세롤 2.25%(장력) 조정제, pH 조절을 위한 수산화 나트륨을 함유하고 있다. Diprivan에는 일반적인 킬레이션제인 EDTA가 포함되어 있으며, 이는 단독으로(일부 박테리아에 대해 세균 정지 효과) 및 일부 다른 항균제와 시너지 효과를 나타낸다. 새로운 일반 제형은 산화 방지제로 메타중아황산나트륨과 항균제로 벤질 알코올을 함유하고 있다. 프로포폴 에멀젼은 유화된 미셀 제형으로부터의 빛 산란으로 인해 불투명한 흰색 액체이다.

프로포폴은 전신 마취 도입에 사용되는 약물로, 거의 완전히 기존 주류 약물이었던 티오펜탈을 대체했다.^[109]

8. 사회적 문제

프로포폴은 사형 절차의 일부로 사용하는 방안이 고려되었지만, 유럽 제조사 및 정부가 이러한 목적으로 프로포폴 수출을 금지하여 실제로 사용된 사례는 없다.^[31][32]

프로포폴의 자가 투여를 통한 오락적 사용이 보고되기도 하지만,^[33][34] 약효가 강력하고 안전한 사용을 위해 필요한 수준의 모니터링이 필요하기 때문에 비교적 드물다. 가파른 용량-반응 곡선은 프로포폴의 오락적 사용을 매우 위험하게 만들며, 자가 투여로 인한 사망 사례가 계속 보고되고 있다.^[35][36] 오락적 사용을 통해 얻고자 하는 단기적 효과에는 가벼운 행복감, 환각, 억제력 상실 등이 있다.^[37][38]

프로포폴의 오락적 사용은 약물에 접근할 수 있는 마취과 의사와 같은 의료진 사이에서 묘사되어 왔다.^[39][40] 일반적으로 사용 시 충분한 휴식을 취한 듯한 느낌을 주기 때문에, 짧은 휴식 기간을 갖는 순환 근무를 하는 마취과 의사들 사이에서 더 흔하다고 한다.^[41] 장기적인 사용은 중독을 초래하는 것으로 보고되었다.^[39][42]

2009년 8월, 로스앤젤레스 카운티 검시관은 음악가 마이클 잭슨이 프로포폴과 벤조디아제핀 계열 약물인 로라제팜, 미다졸람, 디아제팜의 혼합물로 사망했다는 결론을 내리면서 프로포폴의 허가 외 사용 위험에 대한 관심이 증가했다.^{[43][44][45][46]} 2009년 7월 22일 텍사스 주 해리스 카운티 지방 법원에서 공개된 수색 영장에 따르면, 잭슨의 주치의인 콘래드 머레이는 잭슨이 사망하기 직전 리도카인으로 희석된 25mg의 프로포폴을 투여했다.^[44][45][47]

삼성전자 부회장 이재용(2021년),^[206] 미국의 배우 매튜 페리(2022년),^[207] 한국의 배우 유아인(2023년),^{[208][209][210]} 이선균(2023년),^[210] 하정우(2023년)^[211] 등 부적절한 사용에 대한 보도가 끊이지 않고 있다.
