2. 역사

1946년 국제보건회의에서 세계보건기구(WHO) 설립이 허가되었고, 1948년 4월 7일에 정식으로 발족하였다.[360] 이 날은 '세계 보건의 날'로 기념되고 있다.[361]

WHO 설립 이전에도 국제 보건 협력을 위한 노력은 있었다. 1851년부터 1938년까지 개최된 국제위생회의(ISC)는 콜레라, 페스트, 황열병 등 전염병 대응을 논의했다.[13] 회의 결과 1902년 범미보건국과 1907년 국제공중보건사무국이 설립되었다. 1920년에는 국제 연맹이 국제 연맹 보건 기구를 설립했다. 제2차 세계 대전 이후, 국제 연합은 WHO를 설립하면서 기존 보건 기구들을 흡수 통합했다.[17]

1945년 국제연합 국제기구 회의에서 중국 대표 스제밍(Szeming Sze)은 국제 보건 기구 설립을 제안했다. 회의 사무총장 알저 히스(Alger Hiss)의 권고로 국제 보건 회의 소집 선언문이 채택되었다.[18] 1946년 7월 22일 세계보건기구 헌장이 채택되었고,[20] 1948년 4월 7일 헌장이 발효되면서 WHO가 공식 출범했다.[20] 세계보건총회 초대 의장은 안드리야 슈탐파르(Andrija Štampar)였다.

WHO는 말라리아, 결핵, 성병 통제와 모성 건강, 아동 건강, 영양 및 환경 위생 개선을 우선 과제로 삼았다.[23] 1959년 WHO는 국제원자력기구(IAEA)와 협정을 체결하여, 원자력 관련 보건 문제에 대한 조사 권한을 제한받는다는 주장이 제기되기도 했다.[25]

2. 1. 대한민국과 WHO

이종욱 전 WHO 사무총장(2003년 ~ 2006년)은 한국인 최초의 유엔 산하기구 수장으로, 전략보건운영센터(SHOC) 설립, 국제보건협약 제안 등 전염병 대응 체계 강화에 기여했다.[365] WHO는 이종욱 사무총장의 공로를 기려 전략보건운영센터를 '이종욱 전략보건운영센터'(JW Lee Center for Strategic Health Operations)로 명명했다.[366]

3. 목적 및 역할

세계보건기구(WHO)의 목적은 세계 모든 사람들이 가능한 한 최고의 건강 수준에 도달하는 것이다.[360] 이를 위해 WHO는 다음과 같은 두 가지 입헌적 직무를 맡고 있다.[360]

WHO는 세계보건총회, 이사회, 사무국 등 세 가지 주요 기관을 통해 업무를 수행하며, 스위스 제네바에 본부를 두고 있다. 또한, 세계 각지에 6개 지역 기구를 두어 지역별 특성에 맞는 보건 사업을 추진하고 있다.[360]

4. 구조

세계보건기구는 유엔개발그룹의 회원이다.[175] 최고 의사결정 기관은 매년 개최되는 세계보건총회이며, 모든 회원국 대표가 참석한다. 총회에서는 3분의 2 다수결로 조약이나 협정을 제정하며, 회원국은 18개월 이내에 국내 채택 조치를 취해야 한다. 총회는 34개국 위원을 3년 임기로 집행이사회 이사로 선출하며, 이들은 총회의 집행 기관 역할을 한다.[308]

상설 사무국은 총회 의결에 따라 일반 업무를 수행하며, 사무총장이 WHO의 최고 책임자이다. 사무총장은 총회에서 선출된다.[309] WHO 전체 직원 수는 약 8,000명이다.[310]

4. 1. 회원국

세계보건기구 회원국


세계보건기구는 2023년 기준으로 194개 회원국이 가입되어 있다. 리히텐슈타인을 제외한 192개 유엔 회원국, 쿡 제도, 니우에가 정회원이다.[176][177]

푸에르토리코, 토켈라우는 준회원국이다.[178][177] 팔레스타인, 성좌, 몰타 기사단은 옵서버로 참가하고 있다.[179][180]

대만은 2009년부터 2016년까지 중화 타이베이라는 이름으로 옵서버로 참가했으나 양안 관계, 대만 문제로 인하여 참가하지 못하고 있다.[181] 더 뉴스 렌즈에 따르면 대만 기자들은 여러 차례 총회 취재를 거부당했다.[187] 2018년 5월에는 중국의 요구로 대만 언론의 연례 총회 참석이 거부되었다고 한다.[188]

COVID-19 팬데믹으로 인해 대만이 WHO의 팬데믹 관련 긴급 회의에서 배제되면서, 대만의 주요 야당인 국민당을 포함한 대만의 다양한 정당들이 이례적으로 단결하여 대만의 WHO 참여를 허용해야 한다는 압력이 증가했다.[191] 팬데믹 기간 동안 저스틴 트뤼도 캐나다 총리,[192] 아베 신조 일본 총리,[185] 유럽 연합은[193] 코로나바이러스 팬데믹과 관련된 WHO 회의 및 일반적인 참여에 대한 대만의 참여를 지지했다.

4. 2. 세계보건총회(WHA)

세계보건총회(World Health Assembly)는 세계보건기구(WHO)의 입법 및 최고 의결 기구이다. 제네바에 본부를 두고 있으며, 일반적으로 매년 5월에 회의를 개최한다.[207] 총회에는 모든 회원국이 대표를 보낼 수 있다.[307]

총회는 2년제 프로그램 예산을 승인하고 주요 정책 관련 문제를 결정하며, 이는 최종 결정 과정이다.[51] 또한 5년마다 사무총장을 임명하고, 제안된 예산을 포함한 WHO의 정책 및 재정 문제에 대해 투표한다. 집행이사회의 보고서를 검토하고 추가 검토가 필요한 업무 영역이 있는지도 결정한다.[207]

총회에서는 3분의 2 다수결로 조약이나 협정을 제정할 수 있다. 이 조약은 회원국에 대해 구속력은 없지만, 회원국은 자국 대표가 반대한 조약이라도 18개월 이내에 국내 채택을 위해 어떤 조치를 취해야 한다.[308] 총회는 34개국 위원을 3년 임기의 집행이사로 선출하며, 이 집행이사회는 총회의 집행 기관이 된다.[308]

4. 3. 집행이사회(Executive Board)

집행이사회(Executive Board)는 보건 분야의 기술적인 측면에서 적합한 34명으로 구성되어 있으며, 각자 한 개 회원국에 의해 세계보건총회 때에 임명된다.[362] 선출된 회원국의 임기는 3년이며, 각자 임명한 간부는 개인적 능력에 따라 업무를 담당하게 된다.[362]

집행이사회는 매년 적어도 2회 회의를 가지며, 주요 회의는 통상적으로 1월에 열리고, 5월 보건총회가 끝난 직후 두 번째로 더 간략한 회의를 가진다.[362]

집행이사회의 주요 직무는 세계보건총회의 예산 및 결산, 주요 사업 전략 및 운영 방안을 수집하고 검토하는 과정에서 결정과 정책을 실행시키고 충고 및 조장하는 것이다.[362]

4. 4. 사무국

사무국은 본부와 6개 지역사무국, 전 세계에서 근무하고 있는 8,000명 내외의 보건 관련 전문가들로 구성되어 있다. 사무국은 세계보건총회에서 이사회의 동의를 얻어 임명된 사무총장의 지휘를 받는다. 사무총장은 한 명의 사무차장(Deputy Director-General)과 사무총장보(Assistant Director-General)의 보좌를 받는다.[209] 현 사무총장은 2017년 7월 1일에 임명된 테워드로스 아드하놈 거브러이여수스 박사이다.[210]

대한민국 국적의 이종욱 사무총장은 2003년 취임 직후 전략보건운영센터(Centre for Strategic Health Operations)를 만들었다. 2004년 말 완공된 이 센터는 긴급상황 발생 시 30분 안에 관계자 회의를 열 수 있게 했다.[365] 또한 병이 발생한 나라는 의무적으로 WHO에 즉시 보고하도록 하는 '국제보건협약'을 제안했다. 2020년 기준 190여 WHO 회원국이 전염병 발생 즉시 24시간 이내에 WHO 본부에 통보하고 있다.[365]

2005년 조류독감 창궐 당시 이종욱 사무총장은 양치기 소년이라는 비판을 받았으나, "나중에 희생자 숫자가 예상보다 적어 욕을 먹는 한이 있어도 지금 사람들에게 그 위험성을 널리 알려 대비하도록 하기 위함"이라며 신념을 굽히지 않았다.[366] 그는 전염병 전문가를 요직에 발탁하고, WHO의 판데믹 인플루엔자 대응 체제를 재편했다.[366] 2006년 급사 이후, 전략보건운영센터는 '이종욱 전략보건운영센터'(JW Lee Center for Strategic Health Operations)로 불리게 되었다.[366]

4. 5. WHO 지부

각 지역 위원회는 일반적으로 가을에 1년에 한 번 회의를 연다. 각 지역의 모든 회원국 또는 준회원국 대표들이 참석한다. 각 지역에는 지역 사무소가 있으며, 지역 사무소장은 지역위원회가 선출한다.[270] 1999년 이후 지역 사무소장은 5년 임기로 1회 연임이 가능하며, 일반적으로 2월 1일에 취임한다.[271]

WHO의 각 지역위원회는 지역을 구성하는 국가 정부의 모든 보건부 장관으로 구성된다. 지역위원회는 지역 사무소장을 선출하는 것 외에도 세계보건총회에서 채택한 보건 및 기타 정책의 지역 내 이행에 대한 지침을 설정하는 역할을 한다.

6. 비판과 논란

신종 인플루엔자 세계적 유행 당시, 세계보건기구(WHO)의 마거릿 찬 사무총장은 "모든 인류가 위협에 직면해 있다"고 선언하며 신종 인플루엔자를 모든 인류의 위협으로 홍보했다. 이후 신종 인플루엔자의 독성이 약하다는 것이 밝혀졌음에도 불구하고, 심각한 감염 및 사망 피해를 예상한 경고 수준인 6단계/6단계로 경고하며 팬데믹(세계적 대유행)을 선포했다.

하지만 최초로 "국제적으로 우려되는 공중보건 비상사태(PHEIC)" 대상이 된 신종 인플루엔자는 전례 없는 보건 당국과 과학자, 제약회사의 강력한 연계 체제를 만들어냈지만[338], 실제로는 다른 계절성 인플루엔자와 큰 차이가 없는 수준의 인플루엔자였으며 피해 또한 미미했다[339].

WHO의 일련의 오보에 대한 비판이 거세지자[340], 유럽 의회는 팬데믹 선언 경위에 대한 조사에 착수했다. 유럽 의회의 전 보건위원장은 WHO의 선언이 허위 팬데믹이었다고 문제를 제기했다. WHO 의사 결정에 제약회사의 의향이 크게 영향을 미쳤을 가능성이 높다고 지적하며, 제약회사가 연구소 등에서 일하는 과학자들에게 큰 영향력을 행사하고 있으며, WHO가 광범위하게 과학자들의 의견을 구한 것이 그 영향력을 강화하는 원인이 되었다고 말했다. 한편, 신종 인플루엔자 백신 생산 등을 수행하는 세계 최대 규모의 제약회사인 글락소스미스클라인(영국)은 제약회사가 WHO의 팬데믹 선언에 영향을 주었다는 인식은 잘못되었다고 인터뷰에서 답변했다[341].

2010년 1월, 백신이 세계적으로 과잉 상태가 되어 취소 및 재판매가 잇따랐다.

2018년부터 2020년까지 콩고민주공화국에서 WHO 직원들이 성적 학대를 저질렀다는 문제가 불거졌다. 2021년 9월 28일, 사무총장은 사실을 인정하고 사과했다. 문제가 된 직원들은 에볼라 출혈열 대책을 위해 현지 채용된 직원과 각국에서 파견된 직원들이었으며, 지역 주민들에게 일자리를 주는 대가로 성관계를 강요하거나 협박하는 등의 행위를 했다.[357]

6. 1. 국제원자력기구(IAEA)와의 관계

1959년 세계보건기구(WHO)는 국제원자력기구(IAEA)와 세계보건총회(WHA) 12-40 협정에 서명했다. 이 협정에는 "국제원자력기구는 원자력과 관련된 일부 분야에 대한 세계보건기구의 연구나 작업을 저지할 수 있다. 세계보건기구는 보건 증진에 관한 역할을 수행하기 위하여 어떠한 편견 없이 핵에너지의 평화적 이용에 대한 국제원자력기구의 사명을 인정한다"는 내용이 명시되어 있다.[342] 이 때문에 세계보건기구가 체르노빌 원자력 발전소 사고, 후쿠시마 제1 원자력 발전소 사고와 관련된 방사능 문제에 소극적인 입장을 보였다는 비판이 제기되었다. 나카지마 히로시 WHO 사무총장은 “방사선 영향에 대한 연구와 관련하여 WHO는 IAEA에 종속되어 있다. 원자력이 건강을 좌우하고 있는 것이다”라고 말했다.[343]

6. 2. 에볼라, 코로나19 팬데믹 관련 논란

서아프리카 에볼라 유행 당시 세계보건기구(WHO)는 관료주의, 자금 부족, 지역 구조, 직원 배치 문제 등으로 비판받았다. WHO 내부 보고서는 낮은 자금 지원과 개발도상국 의료 시스템의 '핵심 역량' 부족을 주요 문제점으로 지적했다.[365] 마거릿 챈 사무총장은 2015년 세계보건총회에서 비상사태 대응을 위한 1억 달러 규모의 기금을 발표했지만, 2016년 4월까지 모금된 금액은 2,690만 달러에 불과했다. 2016-17년 보건 비상사태 프로그램 예산은 4억 9,400만 달러였으나, 2016년 4월까지 1억 4,000만 달러만 지원되었다.

2019년 코로나바이러스감염증에 의한 급성 호흡기 질환 대응과 관련하여, WHO는 초기 대응 실패와 중국 편향성 논란에 휩싸였다. 특히, 대만이 2019년 12월 인간 간 감염 가능성을 보고했음에도 불구하고, WHO가 대만의 총회 참가를 허용하지 않아 정보가 국제 사회에 제대로 전달되지 않았다는 비판이 제기되었다.[344]

도널드 트럼프 미국 대통령은 WHO가 미국의 막대한 지원에도 불구하고 중국 중심적인 태도를 보이며, 코로나19 팬데믹 대응에 실패했다고 비판했다.[345] 트럼프 대통령은 테워드로스 아드하놈 거브러이여수스 사무총장이 중국의 지지를 받아 당선된 점을 지적하며, 2020년 5월 WHO에 대한 자금 지원 중단을 선언하고, 7월에는 WHO 탈퇴를 공식 통보했다.[348][349]

2021년 조 바이든 대통령 취임 후, 미국은 WHO 탈퇴 결정을 철회하고, 코로나19 백신 공동 구매를 위한 국제 협력체인 COVAX에 참여하는 등 WHO와의 관계를 복원했다.[350][351]

7. 재정

세계보건기구(WHO)는 회원국이 납부하는 분담금과 외부 기부금으로 재정을 운영한다.[232] 분담금은 회원국의 부와 인구에 따라 결정된다.
