2. 역사

3. 적응증

클로자핀 사용은 무과립구증 발생 위험을 높이며, 이는 치명적일 수 있다. 1970년대 핀란드에서 무과립구증으로 인한 사망 8건이 보고되면서 처음으로 알려졌다.[14] 클로자핀 유발 호중구 감소증(CIN)은 약 3.8%의 사례에서 발생하며, 클로자핀 유발 무과립구증(CIA)은 0.4%에서 발생한다.[143]
이러한 위험 때문에 클로자핀은 의무적인 절대호중구수(ANC) 모니터링과 함께 사용된다.[141] 정기적인 혈액 검사를 통해 호중구 수치 감소가 나타나면, 수치에 따라 모니터링을 강화하거나 클로자핀을 즉시 중단한다.[143][144]
클로자핀 재도전(Clozapine rechallenge)은 클로자핀 복용 중 과립구 감소증을 경험한 환자가 다시 약물 복용을 시작하는 것을 말한다. 리튬이나 과립구 콜로니 자극 인자(G-CSF)를 사용하여 중성구 수치를 유지하는 방법도 있다.[159][160][161][162]
덜 알려져 있지만 생명을 위협할 수 있는 또 다른 부작용은 위장관 운동 저하로, 심한 변비, 분변 매복, 마비성 장폐색, 장폐색, 급성 거대결장, 허혈 또는 괴사로 나타날 수 있다.[169] 클로자핀 유발 위장관 운동 저하는 무과립구증보다 사망률이 더 높다.[171]
그 외 중대한 부작용은 다음과 같다.[226]
- 혈구 장애 (호중구 감소증, 무과립구증, 백혈구 감소증)
- 심근염, 심근병증, 심막염, 심낭액 저류
- 고혈당, 당뇨병성 케토산증, 당뇨병성 혼수
- 악성 증후군
- 간질 발작, 경련, 미오클로누스 발작
- 기립성 저혈압, 실신
- 쇼크, 폐색전증, 심부 정맥 혈전증
- 급성 간염, 간염, 담즙 울체성 황달
- 장폐쇄, 마비성 일레우스
6. 3. 기타 부작용
역행성 사정, 프라이피즘과 같은 성 문제가 보고되었다.[2] 드물게 안와 주위 부종과 혈액 악성 종양이 발생하는 경우도 있다.[133][134] 요실금과 관련이 있지만,[179] 그 징후가 간과될 수 있다.[180] 이러한 부작용은 베타네콜로 개선될 수 있다.[181]클로자핀을 갑작스럽게 중단하면 소화불량, 설사, 메스꺼움/구토, 타액 과다 분비, 다한증, 불면증, 불안과 같은 콜린성 반동 효과가 나타날 수 있다.[182] 또한 심각한 운동 장애, 긴장증, 정신병을 유발할 수도 있다.[183] 따라서 환자, 가족, 보호자는 클로자핀의 갑작스러운 중단 증상과 위험을 인지해야 하며, 중단 시에는 금단 증상의 강도를 줄이기 위해 점진적으로 감량하는 것이 권장된다.[184][185]
국제적인 약물 부작용 데이터베이스에 따르면, 클로자핀 사용은 폐렴 발생률 및 사망률을 상당히 증가시키는데, 이는 가장 중요한 부작용 중 하나일 수 있다.[189] 그 메커니즘은 아직 밝혀지지 않았지만, 과다 타액 분비 또는 염증 해소에 대한 클로자핀의 영향과 관련된 면역 효과 때문일 것으로 추측된다.[190][191]
7. 사용 시 주의사항
클로자핀은 효과적인 항정신병 약물이지만, 심각한 부작용을 유발할 수 있으므로 사용 시 주의가 필요하다.
클로자핀은 무과립구증 및 호중구 감소증과 같은 혈액학적 부작용을 일으킬 수 있다.[143] 무과립구증은 심각한 감염으로 이어져 사망에 이를 수 있는 위험한 부작용이다.[145] 따라서 클로자핀 투여 전과 투여 중에는 정기적인 혈액 검사를 통해 백혈구 수, 특히 호중구 수를 확인해야 한다.[30]
클로자핀 투여는 일반적으로 병원에서 시작하지만, 지역사회에서도 가능하다.[82][83] 투여 전에는 환자의 상태를 평가하고 기준 검사를 실시한다. 심근염 발생 가능성을 확인하기 위해 심전도(ECG) 검사나 심초음파 검사를 시행할 수 있다.[85]
초기 용량은 6.5~12.5mg/일로 낮게 시작하여 수 주에 걸쳐 점차 증량한다. 하루 250~350mg 범위에 도달하면 반응 평가를 수행한다.[85] 환자마다 반응이 다르므로, 낮은 용량에서도 효과를 보이는 경우가 있다. 클로자핀 대사에는 인종 간 차이가 있을 수 있는데, 사하라 사막 이남 아프리카계 사람들은 유럽계보다 클로자핀 대사가 빠르고, 동아시아 또는 남서아시아계 사람들은 느린 경향이 있다는 연구 결과가 있다.[89]
플루복사민,[194] 카르바마제핀,[195][196][197] 시프로플록사신[198][199] 등은 클로자핀과 상호작용하여 혈중 농도를 변화시킬 수 있으므로 병용 시 주의해야 한다. 특히, 골수 억제 가능성이 있는 약물, 방사선 치료, 화학 요법 등과는 병용 시 무과립구증 발생 위험이 증가할 수 있다. 장효성 항정신병 약물 (할로페리돌 데칸산 에스터 주사액, 플루페나진 데칸산 에스터 주사액, 리스페리돈 장효성 현탁 주사액)이나 아드레날린 작용 약물(에피네프린, 노르에피네프린)과는 병용을 금지한다.
한국에서는 치료 저항성 조현병 환자에게 클로자핀을 처방할 때 클로자릴 환자 모니터링 서비스(CPMS)에 등록해야 하며,[222] 초기에는 최대 14일분까지 처방 가능했으나, 2021년 6월부터 투여 52주 이후에는 최대 28일까지 처방이 가능하도록 변경되었다.[221] 일본에서도 클로자핀 사용 의료기관, 의사, 약국은 사전에 CPMS에 등록해야 한다.[222]
7. 1. 모니터링
클로자핀 투여 전과 투여 중에는 정기적인 혈액 검사를 통해 백혈구 수치, 특히 호중구 수치를 확인해야 한다.[30] 이는 클로자핀이 호중구 감소증 및 무과립구증을 유발할 수 있기 때문이다.[143] 무과립구증은 심각한 경우 감염으로 인해 사망에 이를 수 있는 위험한 부작용이다.[145]클로자핀 유발 호중구 감소증(CIN)은 약 3.8%의 환자에게서 발생하며, 클로자핀 유발 무과립구증(CIA)은 0.4%에서 발생한다.[143] 이러한 위험을 줄이기 위해, 미국에서는 위험 평가 및 완화 전략(REMS)과 같은 절대호중구수(ANC) 모니터링 시스템을 의무적으로 시행하고 있다.[141]
혈액 검사 빈도와 기준은 국가마다 다르다.[127] 미국, 영국, 오스트랄라시아 등 여러 국가에서 클로자핀 사용 기준과 모니터링 방식에 차이가 있으며,[142] 아이슬란드와 일본은 극단적인 차이를 보인다.[152] 2015년 미국 식품의약국(FDA)은 ANC 기준치를 낮추고 백혈구, 호산구, 혈소판 모니터링 요건을 삭제했다.[152]
치료 초기 18주 동안은 부작용 발생 위험이 가장 높으므로, 이 기간 동안 집중적인 모니터링이 필요하다.[143] 1년 이후에는 무과립구증 발생 위험이 현저히 감소한다.[144] 정기적인 혈액 검사에서 호중구 수치 감소가 확인되면, 정도에 따라 모니터링을 강화하거나 클로자핀 투여를 즉시 중단해야 한다.[143]
클로자핀 투여가 중단된 후 다시 투여를 시작하는 경우(재도전)에는, 이전의 호중구 수치 감소 정도와 국가별 기준을 고려하여 신중하게 결정해야 한다.[24]

7. 2. 투여 시작 및 용량 조절
클로자핀 투여는 일반적으로 병원에서 시작하지만, 지역사회에서도 시작할 수 있다.[82][83] 투여 시작 전에는 여러 평가와 기준 검사가 이루어진다. 영국과 아일랜드에서는 환자가 치료 저항성 조현병, 다른 항정신병제의 부작용으로 인한 불내성, 파킨슨병의 정신병 등의 기준을 충족하는지 평가한다.[84] 치료 저항성을 확인하기 위해 이전 약물 치료 이력을 검토하고, 진단 검토도 수행될 수 있다.[84]기준 검사에는 신체 검사, 신기능 및 간기능 평가, 심전도(ECG) 등이 포함되며, 심근염 가능성 모니터링을 위한 검사도 실시할 수 있다.[85] 호주와 뉴질랜드에서는 심초음파 검사도 일반적이다.[85]
클로자핀 용량은 초기에는 낮게 시작하여 수 주에 걸쳐 점차 증가시킨다. 초기 용량은 6.5~12.5 mg/일이며, 단계적으로 증량하여 하루 250~350 mg 범위에 도달하면 반응 평가를 수행한다.[85] 영국의 평균 클로자핀 용량은 450 mg/일이다.[87][88] 그러나 환자마다 반응이 다양하며, 일부는 낮은 용량에서도 효과를 보인다.

7. 3. 약물 상호작용
플루복사민은 클로자핀의 대사를 억제하여 클로자핀의 혈중 농도를 상당히 증가시킨다.[194]카르바마제핀을 클로자핀과 함께 사용하면 클로자핀의 혈장 농도가 현저히 감소하여 클로자핀의 유익한 효과가 감소한다.[195][196] 카르바마제핀을 시작하거나 용량을 늘리면 환자에게서 "클로자핀의 치료 효과 감소" 여부를 모니터링해야 한다. 카르바마제핀을 중단하거나 카르바마제핀의 용량을 줄이면 클로자핀의 치료 효과를 모니터링해야 한다. 이 연구는 과립구 감소증의 위험 증가로 인해 카르바마제핀을 클로자핀과 함께 사용하지 않는 것이 좋다고 권장한다.[197]
시프로플록사신은 CYP1A2의 억제제이며 클로자핀은 주요 CYP1A2 기질이다. 무작위 연구에 따르면 시프로플록사신을 동시에 복용하는 대상자에서 클로자핀 농도가 상승하는 것으로 보고되었다.[198] 따라서 클로자핀 처방 정보에서는 시프로플록사신 및 기타 CYP1A2 억제제를 치료에 추가할 경우 "클로자핀 용량을 원래 용량의 3분의 1로 줄일 것"을 권장하지만, 시프로플록사신을 치료에서 제거하면 클로자핀을 원래 용량으로 되돌리는 것이 좋다.[199]
골수억제를 일으킬 가능성이 있는 약물, 방사선 치료, 화학요법과 병용하면 무과립구증 발현 증가의 우려 및 혈액장애 부작용이 상호 증강될 가능성이 있다.
장효성 항정신병 약물은 혈중에서 소실되는 데 시간이 걸려 부작용에 대처할 수 없기 때문에 병용을 금지한다.
아드레날린 작용 약물은 아드레날린의 작용을 역전시켜 중증의 저혈압을 일으킬 우려가 있어 병용을 금지한다.

더불어민주당 등 진보 진영에서는 정신 질환 치료에 대한 사회적 지원 강화와 함께 클로자핀과 같은 효과적인 약물 치료 접근성을 높이는 정책을 추진하고 있다.
8. 1. 클로자핀 사용의 인종 간 차이
의료 서비스 제공에서 소수 민족이 열등한 치료를 받는다는 일반적인 사실이 있으며, 이는 특히 미국에서 두드러진다.[119][120][121][122] 미국에서는 흑인이 백인에 비해 2세대 항정신병약(대체 약물보다 비싼)을 처방받을 가능성이 낮다는 일반적인 사실이 있으며, 이는 특히 재향군인부 의료 시스템에서 클로자핀과 관련하여 사회경제적 요인의 차이를 고려할 때 더욱 분명했다.[104][105][123] 흑인 환자는 클로자핀 복용을 시작할 가능성이 낮을 뿐만 아니라, 클로자핀 복용을 중단할 가능성이 더 높은데, 이는 아마도 양성 인종성 호중구 감소증 때문일 수 있다.참조
