3. 부작용

1~10% 발생률로 비교적 흔한 부작용에는 설사, 체중 감소, 구역질, 두통, 불면증, 식욕부진, 복통, 비염, 부비동염, 요로감염증, 우울증 등이 있다.^{[39][49][6][13][14][15][16]}

로플루밀라스트는 포스포다이에스터레이스-4 억제제로, 효소의 일종인 포스포다이에스터레이스-4(PDE-4)에 직접 결합해 효과를 차단한다. PDE-4는 COPD의 발병 기전에 관여하는 다양한 염증성 세포에서 발현하는 것으로 알려져 있다. 로플루밀라스트는 직접적인 기관지확장제로서 효과를 보이지는 않으나, 만성 염증을 경감시키는 효과가 있으므로 이미 사용 중인 기관지확장제에 추가해서 보조 약제로서 주로 사용한다.^[50]

로플루밀라스트의 임상시험 결과는 다음과 같다. 2015년에 보고된 위약 대조 무작위 비교 시험(REACT 시험)^[31]에서, 흡입 스테로이드(ICS)와 장시간 작용성 β 작용제(LABA), 장시간 작용형 항콜린제(LAMA)를 사용 중인 중등증~중증 만성 폐쇄성 폐 질환(COPD) 환자를 대상으로 한 연구 결과, 악화의 연간 발생 횟수는 로플루밀라스트군과 위약군 사이에 유의미한 차이가 없었다.^[31] 그러나 중증 악화 빈도 및 악화로 인한 입원율은 로플루밀라스트군에서 유의미하게 낮았다.^[32] 설사 부작용은 로플루밀라스트군에서 더 흔하게 나타났다.^[33]

로플루밀라스트는 유럽 연합에서 2010년 6월, 미국 식품의약국(FDA)에서 2011년 2월에 각각 승인되었다.^{[5][17][18][19]} 유럽 연합에서는 만성 기관지염과 관련된 중증 만성 폐쇄성 폐 질환(COPD)에 대해 승인되었고, 미국 FDA에서는 COPD 악화를 줄이는 용도로 승인되었다.
