3. 의학적 용도

할로테인은 최소 폐포 농도가 0.74%로, 마취 유도 및 회복 시간이 중간 정도인 강력한 마취제이다.^{[14][15][104]} 혈액/가스 분배 계수는 2.4이다.^[15] 진통 효과는 미미하며, 근육 이완 효과는 중간 정도이다.^[16][61]

4. 약리 작용

최소 폐포 농도(MAC)는 0.74%로, 마취 작용 시작과 회복 속도가 중간 정도이다. 진통 및 근육 이완 효과 역시 중간 정도이다.^{[104][14][16]} 혈액/가스 분배 계수가 2.4이므로, 마취 유도 및 회복 시간은 중간 정도이다.^[15]

전신 마취제의 정확한 작용 기전은 아직 밝혀지지 않았다.^[29] 할로테인은 GABA_A 수용체와 글리신 수용체를 활성화한다.^[30][31] 또한 NMDA 수용체 길항제로 작용하며,^[31] 니코틴성 아세틸콜린 수용체와 전압 개폐 나트륨 채널을 억제하고,^[30][32] 5-HT3 수용체 및 twin-pore K⁺ 채널을 활성화한다.^[30][33] AMPA 수용체 또는 카이네이트 수용체에는 영향을 미치지 않는다.^[31]

5. 부작용

할로세인의 부작용은 다음과 같다.

• 심혈관계: 부정맥, 심근 억제, 저혈압 등이 발생할 수 있다. 특히 카테콜아민에 대한 심장 민감도를 증가시켜 부정맥 위험을 높일 수 있으며, 이 경우 치명적인 심장 부정맥을 유발할 수 있다.^[25] 치과 마취에서 특히 문제가 되는 것으로 보인다.^[25] 소아를 대상으로 한 무작위 대조 시험에서 부정맥 발생률이 세보플루란(28%)보다 할로테인(62%)에서 유의하게 높았다.^[74]
• 호흡기계: 호흡 억제가 발생할 수 있다.^[5]
• 간: 드물게 간독성(할로테인 간염)이 발생할 수 있으며, 반복 노출 시 위험이 증가한다.^[5] 이는 약 10,000번 노출당 1번 꼴로 발생한다.^[18] 할로테인 간염은 면역 알레르기성 기전으로,^[18] 간에서 산화 반응을 통해 할로테인이 트리플루오로아세트산으로 대사되기 때문에 발생하는 것으로 생각된다. 흡입된 할로테인의 약 20%는 간에서 대사되며, 이 대사 산물은 소변으로 배설된다. 할로테인 간염의 사망률은 30%에서 70%에 이른다.^[19] 간염에 대한 우려로 인해 1980년대에 성인에게 할로테인의 사용이 급격히 감소했으며, 아이소플루란과 엔플루란으로 대체되었다.^[20][21]
• 악성 고열증: 유전적 소인이 있는 환자에게서 드물게 발생할 수 있는 치명적인 부작용이다.^[5]
• 기타: 자궁 평활근 이완으로 인한 분만 또는 임신 중절 시 출혈량 증가^[26], 포르피린증 환자에게는 비교적 안전한 것으로 알려져 있다.^[6] 임신 중 사용이 아기에게 해로운지 여부는 불분명하며, 일반적으로 제왕 절개 중에는 사용이 권장되지 않는다.^[11]

할로세인(2-브로모-2-클로로-1,1,1-트라이플루오로에테인)은 밀도가 높고 휘발성이 강하며, 투명하고 무색이며, 가연성이 없는 액체로, 클로로포름과 유사한 달콤한 냄새가 난다. 물에는 매우 약간 용해되며 다양한 유기 용매와 혼화성이 있다. 할로세인은 빛과 열에 의해 플루오린화 수소, 염화 수소, 브로민화 수소로 분해될 수 있다.^[34]

화학적으로, 할로세인은 많은 다른 마취제와 달리 알킬 할라이드(에테르가 아님)이다.^[1] 이 구조는 하나의 입체 중심을 가지고 있어 (''R'')- 및 (''S'')-광학 이성질체가 존재한다. 할로세인은 키랄 분자이며, 라세미체로 사용된다.^[84]

트리클로로에틸렌을 이용한 할로테인의 상업적 합성이 이루어진다. 엔플루란과 이소플루란은 할로테인과 유사한 흡입 마취제이지만 간독성 위험이 더 낮다. 세보플루란과 데스플루란과 같은 더 현대적인 흡입 마취제는 마취 유도 및 회복이 더 빠르다.^[39]

세계 보건 기구의 필수 의약품 목록에 등재되어 있다.^[9] 휘발성 액체 형태로 용기당 30, 50, 200, 250ml로 제공되지만, 많은 선진국에서는 새로운 제제에 의해 대체되어 더 이상 사용되지 않는다.^[46]

할로세인은 공유 결합된 플루오린(F) 원자를 포함하여 대기의 창에서 흡광을 나타내므로 온실 가스이다. 그러나 대기 중 수명이 약 1년으로 짧아, 100년 이상인 다른 염화플루오린화탄소 및 브롬플루오린화탄소보다 훨씬 덜 강력하다.^[48] 짧은 수명에도 불구하고, 할로세인은 여전히 이산화탄소의 47배에 달하는 지구 온난화 지수를 가지고 있지만, 이는 가장 풍부한 불소화 가스보다 100배 이상 작고, 500년 동안 육불화황의 지구 온난화 지수보다 약 800배 작다.^[49] 할로세인은 지구 온난화에 무시할 만한 기여를 하는 것으로 여겨진다.^[48]

할로세인은 오존 파괴 물질이며, ODP는 1.56이고, 전체 성층권 오존층 파괴의 1%를 차지하는 것으로 추산된다.^[12][13]

사람들은 직장에서 폐기 마취 가스를 흡입하거나, 피부 접촉, 눈 접촉, 또는 삼킴으로써 할로테인에 노출될 수 있다.^[27] 미국 국립 산업안전보건연구원(NIOSH)은 60분 동안 2 ppm (16.2mg/m3)의 권고 노출 기준(REL)을 설정했다.^[28]
