2. 의학적 사용

리툭시맙은 B세포 표면 항원인 CD20을 표적으로 하는 키메라 단클론 항체이다. 리툭시맙은 정상 B세포와 병원성 B세포를 모두 고갈시키지만, CD20 항원을 발현하지 않는 형질세포와 조혈모세포는 보존하는 방식으로 작용한다.[126]

리툭시맙은 전 세계적으로 다양한 질환 치료에 사용되고 있으며, 국가별로 승인된 적응증에는 차이가 있다.

2. 1. 혈액암

리툭시맙은 백혈병과 림프종을 포함하는 혈액암 치료에 사용된다. 구체적으로는 비호지킨 림프종(NHL), 만성 림프구성 백혈병(CLL), 결절성 림프구 우세형 호지킨 림프종 등이 있다.[128][129] 발덴스트롬 거대글로불린혈증(비호지킨 림프종의 일종)도 리툭시맙으로 치료할 수 있다.[120]

히알루로니데이스와 리툭시맙을 섞은 약은 'Mabthera SC', 'Rituxan Hycela' 등의 이름으로 판매되며,[130] 소포림프종, 미만성 거대 B세포 림프종(DLBCL), 만성 림프구성 백혈병 치료에 사용된다.[130] CD20 양성 만성 림프구성 백혈병에는 플루다라빈, 사이클로포스파마이드와 함께 사용하기도 한다.[131]

소아 환자의 경우, 이전에 치료받지 않은 진행성 CD20 양성 미만성 거대 B세포 림프종(DLBCL), 버킷림프종(BL), 버킷유사림프종(BLL), 성숙 B세포 급성 백혈병(B-AL)에 대하여 화학요법과 병용 투여할 수 있다.[127]

2. 2. 자가면역질환

리툭시맙은 여러 자가면역질환 치료에 사용된다. 류마티스 관절염의 경우, 특히 하나 이상의 TNF 억제제 치료에 불충분한 반응을 보인 환자들에게 효과적이다.[131] FDA는 메토트렉세이트(MTX)와 병용하여 중등도에서 중증의 활동성 류마티스 관절염 환자에게 사용하도록 승인했다.[133] EU에서는 메토트렉세이트와의 조합은 하나 이상의 TNF 억제제 치료에 불충분한 반응을 보인 중증의 활동성 류마티스 관절염 환자에게 사용하도록 승인되어, 미국보다 사용 범위가 더 제한적이다.[133]

다발혈관염 동반 육아종증, 현미경적 다발혈관염과 같은 혈관염[131]과 중등도에서 중증의 보통천포창[131] 치료에도 사용된다.

이 외에도 전신 홍반 루푸스,[136] 다발성 경화증,[134][135] 만성 염증성 탈수초성 다발신경병증[136] 등 치료가 어려운 여러 자가면역질환에 오프라벨로 널리 사용된다.[136]

다른 자가면역질환 치료에도 사용되는데, 자가면역성 용혈성 빈혈, 순적혈구 무형성증, 혈전성 혈소판감소성 자반증(TTP),[138] 특발성 혈소판감소성 자반증(ITP),[139][140] 에반스 증후군,[141] 다발혈관염 동반 육아종증과 같은 혈관염, 천포창이나 유사천포창 등의 물집 피부병,[142] 제1형 당뇨병, 쇼그렌 증후군, 항NMDA 수용체 뇌염, 시신경척수염(데빅병),[143] 그레이브스 안병증,[144] 자가면역성 췌장염,[145] 안구간대경련-근간대경련 증후군(OMS),[146] IgG4 관련 질환[147] 등이 있다. 다만, IgA 매개 자가면역질환 치료에는 효과가 적다는 증거가 있다.[148]

3. 작용 기전

리툭시맙은 세포 표면에 있는 CD20 단백질에 결합하는 항체이다. CD20은 초기 pre-B 세포부터 성숙 B세포까지는 발현되지만, 최종 분화된 형질세포에서는 발현되지 않는다. CD20의 기능은 완전히 알려지지는 않았지만, 칼슘 이온(Ca2+)이 세포막을 통과하여 세포 내로 들어오게 하여 B세포 활성화에 관여하는 것으로 추정된다.

리툭시맙은 CD20을 세포 표면에 적게 발현하는 세포를 제거하는 데에는 비교적 효과적이지 못하다.[150] 리툭시맙은 CD20의 한쪽에 달라붙는 경향이 있으며, 이때 캡(cap)을 형성하여 결합한 쪽으로 단백질을 끌어당긴다. 캡이 있으면 자연살해세포(NK세포)가 B세포를 파괴하는 성공률이 올라간다. NK세포가 캡에 붙으면 B세포를 죽이는 성공률은 80% 정도이나, B세포가 이러한 비대칭적인 단백질 클러스터를 가지고 있지 않으면 성공률은 40%에 그친다.[151]

현재까지 알려진 리툭시맙의 효과는 다음과 같다.[152]

  • 리툭시맙의 Fc 부위는 항체의존성 세포매개독성(ADCC)와 보체의존성 세포독성(CDC)을 매개한다.[153]
  • 리툭시맙은 세포 주기에 광범위한 조절 효과를 보인다.
  • 특히 만성 림프구성 백혈병(CLL)에서, CD20 발현 수준이 높으며 BCR 신호전달 경향이 강한 악성 B세포를 더 잘 제거한다.[150]
  • MHC 클래스 II와 부착 분자인 LFA-1, LFA-3을 상승시킨다.
  • CD23의 배출을 일으킨다.
  • B세포 수용체를 하향조절한다.
  • CD20+ 세포의 세포자멸사를 유도한다.
  • 일부 만성 림프구성 백혈병 세포가 종양 미세환경으로부터 방출되도록 유도한다. 이는 리툭시맙과 함께 사용되는 화학요법에 악성 세포가 더 민감하게 노출되도록 만들 수 있다.[153]


몸에서 악성 B세포를 포함한 B세포가 제거되면서 여러 복잡한 효과가 발생하며, 새로운 건강한 B세포가 림프계 줄기세포로부터 발생한다.

리툭시맙이 결합하는 부위는 CD20의 170 ~ 173번과 182 ~ 185번 아미노산으로, 이 두 아미노산 서열은 167번과 183번 아미노산이 이황화 결합으로 결합해 있기 때문에 물리적으로 서로 가까운 위치에 있다.[154]

4. 부작용

리툭시맙은 B세포를 고갈시키기 때문에 면역 억제와 관련된 다양한 부작용이 나타날 수 있다. 사망이나 영구적인 장애로 이어질 수 있는 심각한 부작용으로는 중증 주입관련반응, 심정지, 사이토카인 방출 증후군, 종양 용해 증후군으로 인한 급성 콩팥 손상, 감염, 면역 독성, 폐 독성, 장폐색과 장천공 등이 있다.[131][120] 특히, B형 간염바이러스 보균 환자의 경우 리툭시맙 투여로 인해 면역이 억제되어 바이러스가 활성화되면서 극심성 간염이나 간염 악화를 일으킬 수 있으므로 주의해야 한다.

4. 1. 일반적인 부작용

리툭시맙의 일반적인 부작용은 다음과 같다.[131][120][28][17]

전신 홍반 루푸스나 류마티스 관절염 환자에게서도 PML 발생 사례가 보고되었다.[159][160] 또한, 리툭시맙은 림프종 환자에서 만성 E형 간염의 보조인자일 가능성이 있다고 보고되었다.[161]

리툭시맙을 지속적으로 치료받으면 백신 반응이 잘 나타나지 않을 수 있다.[162][52] 코로나19 범유행 기간 동안 다발성 경화증 환자가 리툭시맙 치료를 받았을 때 중증 코로나19에 걸릴 위험이 더 높다는 사실이 밝혀지기도 했다.[163][53][54]

4. 2. 중증 부작용

리툭시맙의 심각한 부작용은 사망이나 영구적인 장애를 초래할 수 있으며, 다음과 같은 것들이 있다.[131][120]

전신 홍반 루푸스 환자 두 명이 리툭시맙 치료 후 진행성 다초점 백질뇌병증(PML)으로 사망한 사례가 있었다. PML은 뇌에서 잠복감염 상태로 있는 사람 폴리오마바이러스 2가 활성화되면서 발생한다. 사람 폴리오마바이러스 2의 재활성화는 대개 사망이나 심각한 뇌 손상으로 이어진다.[159]

리툭시맙 치료를 받은 류마티스 관절염 환자에서 PML이 발생한 사례가 최소 1건 보고되었다.[160]

리툭시맙은 림프종 환자에서 만성 E형 간염의 보조인자일 가능성이 있다고 보고된 바 있다. E형 간염은 보통 급성 감염으로 나타나는데, 리툭시맙 투여와 림프종이 함께 인체의 바이러스에 대한 면역 반응을 약화시켰을 수 있다.[161]

지속적인 리툭시맙 치료의 주된 걱정거리는 적절한 백신 반응을 유도하기 어려워진다는 것이다.[162] 코로나19 범유행 기간 동안 이 점이 조명을 받았는데, 다발성 경화증 환자가 리툭시맙 치료를 받았을 때 중증 코로나19에 걸릴 위험이 더 높았기 때문이다.[163] 다발성 경화증 환자가 리툭시맙 치료를 받았을 때, B세포 수가 40/µL 이상이었던 환자 중 10명 중 9명이 화이자-바이온텍 코로나19 백신 접종 이후 항체가 보호 수준에 도달하였다.[164] B형 간염바이러스 보균 환자에서 본제의 투여로 인한 면역 억제에 의한 바이러스 활성화로 극심성 간염 또는 간염 악화를 일으킬 수 있다.

5. 역사

리툭시맙은 1991년 미국의 IDEC Pharmaceuticals(현 바이오젠)에서 개발한 항체 의약품이다.[101] 인간 림프구 B 세포에만 발현되는 CD20을 표적으로 하며, 정상 세포와 종양 세포를 가리지 않고 preB에서 성숙 B 세포까지 세포막 표면에서 관찰된다. preB, 형질 세포에는 CD20이 관찰되지 않는다. 마우스의 인간 CD20에 대한 항체의 가변 영역 Fab와 인간의 불변 영역 Fc를 키메라로 만들었다.[101]

1997년 미국 식품의약국(FDA)에서 B세포 비호지킨 림프종 치료제로 처음 승인되었다.[64][65] 이후 미만성 거대 B세포 림프종 등 다양한 B세포 림프종 치료에 CHOP 화학요법과 함께 사용되고 있다.[66] 2010년에는 유럽 위원회에서 여포 림프종 유지 요법으로 승인받았다.[67]

2016년에는 B-세포 만성 림프구성 백혈병/림프종(CLL)에 대한 피하 주사 제형으로 EU 승인을 받았다.[68] 2017년 미국 FDA는 성인 여포 림프종, 미만성 거대 B세포 림프종 및 만성 림프구성 백혈병 환자를 위한 리툭시맙과 히알루론산 인간(상품명: Rituxan Hycela)의 병용 요법을 승인했다.[69]

2019년 미국 FDA는 2세 이상 소아에서 육아종증 및 현미경적 다발혈관염 치료를 위해 리툭시맙 주사를 글루코코르티코이드(스테로이드 호르몬)와 함께 사용하는 것을 승인했다.[70] 2021년 미국 FDA는 이전에 치료받지 않은 진행성 CD20 양성 미만성 거대 B세포 림프종, 버킷 림프종, 버킷 유사 림프종 또는 성숙 B세포 급성 백혈병을 앓고 있는 6개월에서 18세 사이 소아 환자를 위한 화학 요법과 리툭시맙 병용 요법을 승인했다.[24][71]

리툭시맙은 ANCA 관련 혈관염 치료제로도 사용되며, 호산구성 다발 혈관염성 육아종증에도 효과를 보인다.[102] 장기 이식 시 거부 반응 치료, 류마티스 관절염, 전신 홍반 루푸스(SLE) 등 난치성 자가 면역 질환 치료에도 사용되고 있다.[104]

일본에서 건강 보험 적용 대상은 CD20 양성 B 세포성 비호지킨 림프종, 면역 억제 상태에서의 CD20 양성 B 세포성 림프 증식성 질환, 다발 혈관염성 육아종증, 현미경적 다발 혈관염이다. 비호지킨 림프종, 림프 증식성 질환 치료에는 단독으로 사용되기도 하지만, CHOP 요법과 병용하기도 한다.[101]

리툭시맙은 세계 보건 기구의 필수 의약품 목록에 등재되어 있다.[20]

6. 사회와 문화

2024년 7월, 유럽 의약품청(EMA)의 의약품 위원회는 Dr. Reddy's Laboratories/Holding GmbH가 비호지킨 림프종, 만성 림프구성 백혈병, 류마티스 관절염, 육아종증 및 현미경적 다발혈관염, 그리고 천포창 치료를 위해 개발한 리툭시맙 바이오시밀러 Ituxredi에 대한 시판 허가를 권고하는 긍정적인 의견을 채택했다.[72] Ituxredi는 2024년 9월에 유럽 연합에서 의학적 사용을 위해 승인되었다.[72][73] Dr. Reddy's의 리툭시맙 바이오시밀러 Reditux는 2007년에 인도에서 승인되었다.[74]

7. 관련 연구

2009년, 암 치료를 위해 메토트렉세이트로 유도된 B 세포 고갈 치료를 받던 환자가 근육통성 뇌척수염/만성 피로 증후군(ME/CFS) 증상의 일시적인 완화를 경험했다. 리툭시맙을 사용한 초기 임상 시험은 유망했지만, 2019년에 발표된 3상 임상 시험에서는 리툭시맙 치료와 ME/CFS 개선 사이의 연관성을 찾지 못했다.[92][93]

지주막하강 내에 리툭시맙을 투여하여 CNS 질환을 치료하는 가능성에 대한 연구가 진행 중이다.[94]
