2. 효능 및 효과

네비라핀은 6세 이상의 HIV-1 감염 환자에게 다른 항레트로바이러스 약제와 함께 사용된다. 단독 요법은 내성이 생기기 쉬워 권고되지 않는다.[71][72] 보통 CD4 세포 수가 매우 낮을 때에만 사용이 고려된다.[71] 낮은 CD4 세포 수나 높은 바이러스 수치를 가진 환자에게 네비라핀 사용에 대한 우려가 있지만, 일부 분석에서는 이러한 환자들에게도 네비라핀이 효과적이라고 주장한다.[76]

아이들의 용량은 체표면적(BSA)에 따라 결정되지만,[71] 최근에는 체중 기반 용량 알고리즘이 제시되었다. 이 가이드라인은 아주 어린 신생아에게도 적용할 수 있다.[77]

2. 1. HIV-1 감염 치료

네비라핀을 포함한 3제 요법은 항레트로바이러스 치료 경험이 없는 환자에게서 바이러스 억제에 효과적이다.[73] 일부 임상시험에서는 네비라핀 기반 치료법이 단백질분해효소 억제제[74][75]나 에파비렌즈[76] 기반 치료법과 유사하거나 더 뛰어난 효과를 보였다. 네비라핀은 바이러스학적 실패 이후, 특히 이전에 NNRTI를 복용한 적이 없는 환자에게 구제 요법의 유용한 구성 요소가 될 수 있다.

2. 2. HIV 수직 감염 예방

우간다와 태국에서 실시된 임상시험 결과, 산모와 신생아가 네비라핀을 복용하면 HIV 수직 감염률을 지도부딘에 비해 약 50%까지 낮출 수 있었다.[78][11][41] 태국에서 이루어진 연구에서는 네비라핀과 지도부딘을 함께 예방 목적으로 복용하는 것이 지도부딘만 사용하는 것보다 훨씬 효과적이었다.[79][13][42]

이러한 임상시험 결과를 바탕으로 WHO는 많은 개발도상국에서 모자 간 감염을 줄이는 비용 효율적인 방법으로 네비라핀 단일 용량 예방 요법 사용을 권장하게 되었다. 그러나 미국에서는 우간다 연구에 결함이 있다는 사실이 밝혀지면서,[80][14][43] 2006년 FDA은 네비라핀을 예방 목적으로 사용하는 것을 승인하지 않았다.[81][15][44]

하지만 HIVNET 012 실험 지지자들은 실험의 결함이 관료주의적인 문제 때문이라고 주장하며, 네비라핀의 안전성과 효능에 관한 연구 결과는 과학적으로 확실하므로 무시할 수 없다고 주장했다.[82][16][45] 또한, 자원이 부족한 아프리카 연구자들에게 서구 연구자들과 같은 기준을 적용하는 것은 비현실적이며, 아프리카 과학 발전을 저해할 것이라고 주장했다.[83][17][46]

2013년 9월에는 수유 기간 동안 HIV 수직 감염 예방에 네비라핀을 사용할 수 있는지에 대한 임상시험이 이루어졌다.[84][18]

이러한 논란이 발생하는 이유는 네비라핀을 복용하면 산모와 태아 모두에게서 NNRTI(non-nucleoside reverse transcriptase inhibitor) 약물에 대한 내성이 발견되기 때문이며,[85][19][50] 이는 향후 NNRTI를 포함하는 치료법에 부정적인 영향을 끼칠 수 있다.[86][20][51] 이러한 위험을 줄이기 위해 미국 공중 보건 서비스 태스크 포스는 라미부딘/지도부딘 사용을 권장하고 있다.[87][26][59]

3. 부작용

네비라핀의 가장 흔한 부작용은 경미하거나 중간 정도의 발진(13%)이다.[88][89][33][22] 스티븐스-존슨 증후군, 독성 표피 괴사 용해증, 과민 반응을 포함한 중증 또는 생명을 위협하는 피부 반응은 환자의 1.5%에서 관찰되었다.[88][33]

네비라핀은 특히 치료 시작 후 6주 이내에 중증 또는 생명을 위협하는 간독성을 보일 수 있다.[88][90][33][23] 2000년, 미국 식품의약국(FDA)은 네비라핀에 블랙 박스 경고를 표기하고, 이 약이 간독성과 피부 반응에 생명을 위협할 정도의 독성을 나타낼 수 있다는 사실을 경고했다.[91][6][56] 특정 집단의 환자(여성의 CD4 수>250, 남성의 CD4 수>400)에게서 나타나는 심각한 수준의 간 질환으로 인해[9][24] 미국 보건복지부(DHHS)는 환자들이 네비라핀을 복용함으로써 얻는 이익이 크더라도, 위험 요인이 낮은 환자들에게만 사용하도록 제한했다.[90][23][55] 그러나 이러한 CD4 제한을 발견한 2NN 연구에서 그 효과는 태국에서 모집된 환자에게서만 나타났다.[25][58] 미국 질병예방 특별위원회는 네비라핀을 임신 중 사용하는 것은 네비라핀의 독성을 더욱 악화시키기 때문에 주의해야 한다고 주장한다.[92][26][59]

면역 재구성 증후군 및 지방 재분포 역시 네비라핀과 함께 사용하는 경우 관찰된 부작용이다.[91][6]

미국 FDA는 다음과 같은 증상이 나타날 때 네비라핀 투여를 중단하라고 권고한다.[91][6]


  • 간염과 같은 간 질환 관련 증상이나 신호
  • 발진이나 전신적 증상 및 전이 효소의 증가
  • 발진과 전신적 증상
  • 심각한 피부 반응 및 과민 반응


또한 미국 FDA는 초기 6주 동안은 이 시기에 부작용이 발생할 확률이 높으므로 환자가 위의 증상들이 발생하지 않는지 면밀히 모니터링할 것을 권고한다.[91][6] 지속적인 모니터링은 초기 18주까지 지속되어야 하며, 만약 환자가 간염이나 발진, 과민 반응 또는 다른 전신적 증상을 호소하면 네비라핀을 즉시 종료하고 다시 사용해서는 안 된다.[91][6]

임상 시험 및 시판 후 임상 시험에서 확인된 부작용은 총 36.6%였으며[52], 주요 부작용으로는 발진, 구역, 구토, 두통, 졸음, 피로, 발열 등이 있었다.[53][63][54]

심각한 부작용은 다음과 같다.[52][63]

  • 독성 표피 괴사 융해증（0.1%), 스티븐스-존슨 증후군(0.7%), 과민성 증후군
  • 간염(극성 간염 포함), 간 기능 장애(AST(GOT), ALT(GPT), γ-GTP, Al-P, 총 빌리루빈 등의 상승), 황달, 간부전(11.40%)
  • 과립구감소증, 우울증, 환각, 혼란, 탈수, 심근 경색, 출혈성 식도 궤양, 전신 경련, 수막염(3.77%)
  • 아나필락시스 유사 증상（0.36%）

4. 약물 상호작용

네비라핀은 간 효소 CYP3A와 CYP2B6의 기질이므로, 이 효소들을 억제하거나 유도하는 약물과 상호작용할 수 있다.[93] 리토나비르, 포삼프레나비르, 플루코나졸과 같은 효소 억제제는 네비라핀의 혈중 농도를 증가시킬 수 있으며,[93] 리팜피신과 같은 효소 유도제는 네비라핀의 혈중 농도를 감소시킬 수 있다.[94]

세인트존스워트(St. John's wort)는 CYP3A4와 CYP1A2를 유도하여 네비라핀의 혈중 농도를 낮출 수 있으므로 함께 사용하지 않는 것이 좋다.[93]

네비라핀은 CYP3A4와 CYP2B6을 유도하여 에파비렌즈, 인디나비르, 로피나비르, 넬피나비르, 사퀴나비르, 클래리트로마이신, 케토코나졸, 호르몬 피임약, 메타돈 등 다른 약물의 혈중 농도를 감소시킬 수 있다.[95]

5. 작용 기전

네비라핀은 비뉴클레오사이드 역전사 효소 억제제(NNRTI) 계열의 항레트로바이러스제이다.[60] 뉴클레오사이드 및 비뉴클레오사이드 RTI는 모두 바이러스 RNA를 DNA로 전사하는 바이러스 효소인 역전사 효소를 억제한다.[97] 뉴클레오사이드 RTI와는 달리, NNRTI는 활성자리에서 약 10 옹스트롬 떨어진 p66의 하위 도메인에 있는 소수성 포켓(NNRTI 포켓)에 결합한다.[30] 이 NNRTI 결합 포켓은 NRTIs와는 다른 방식으로 역전사를 억제한다.

네비라핀은 HIV-2 역전사 효소의 포켓 구조가 HIV-1과 다르기 때문에 HIV-2에 효과적이지 않다.[31]

바이러스 복제가 완전히 억제되지 않으면 네비라핀에 대한 내성이 빠르게 발생한다.[32] 네비라핀 치료 후 관찰되는 가장 흔한 돌연변이는 Y181C 및 K103N이며, 이는 다른 NNRTI에서도 관찰된다.[33][34] 모든 NNRTI가 동일한 포켓 내에 결합하므로 네비라핀에 내성이 있는 바이러스 균주는 일반적으로 에파비렌즈, 델라비르딘과 같은 다른 NNRTI에도 내성을 보인다. 그러나 릴피비린, 에트라비린과 같은 2세대 NNRTI는 네비라핀 및 동계열의 다른 1세대 약물에 내성이 있는 HIV 균주 치료에 효과적이다.

네비라핀은 Karl D. Hargrave과 베링거인겔하임 그룹의 회사 중 하나인 Boehringer Ingelheim Pharmaceuticals, Inc.의 동료가 함께 발견했다.[105] 미국 식품의약국(FDA)에서 1996년 6월 21일에 최초의 NNRTI로 허가되었다.[107] 1996년 6월에 성인 환자에 대해, 1998년 9월에 소아에 대해 승인되었다. 1997년에는 유럽에서,[108] 1998년 2월에는 유럽에서, 1998년 11월에는 일본에서 승인되었다.

7. 사회와 문화

남아프리카 공화국에서는 치료 행동 캠페인이 네비라핀의 광범위한 보급 실패와 관련하여 정부를 상대로 한 소송에서 승리했다.[109] 남아프리카 공화국 헌법 재판소는 보건부 장관 대 치료 행동 캠페인 사건에서 정부에 다음과 같이 명령했다.[109]

  • 해당 시설의 의료 책임자와 협의하여 진료하는 의료 종사자의 판단에 따라 의학적으로 필요한 경우, 그리고 해당 산모가 적절한 검사를 받고 상담을 받은 경우 병원 및 진료소에서 이를 사용할 수 있도록 할 것.


미국 조지 W. 부시 대통령의 PEPFAR 기금 5억 달러는 아프리카의 에이즈 유행 퇴치를 돕기 위한 것으로, 다른 약물 및 프로그램과 함께 네비라핀을 포함했다.[109]
