2. 역사

부프로피온은 1969년 나리만 메타가 버로스 웰컴(Burroughs Wellcome, 현재 글락소스미스클라인)에서 개발했으며, 1974년 미국 특허를 받았다.[14] 1985년 12월 30일 미국 식품의약국(FDA)에서 항우울제로 승인받아 웰부트린(Wellbutrin)이라는 이름으로 시판되었다.[169][170] 그러나 원래 권장 용량(400–600mg/day)에서 발작 발생률이 높아 1986년에 회수되었다. 이후 발작 위험이 용량 의존적이라는 것이 밝혀졌고, 1989년 1일 최대 권장 용량을 450mg으로 낮춰 재승인되었다.[171]

1996년 미국 식품의약국(FDA)은 1일 2회 복용하는 부프로피온 서방형 제제 웰부트린 SR을 승인했다.[172] 1997년에는 자이반(Zyban)이라는 이름으로 금연 보조제로 FDA 승인을 받았다.[174][172] 2003년, FDA는 1일 1회 복용하는 또 다른 서방형 제제 웰부트린 XL을 승인했다.[173] 2006년, 웰부트린 XL은 계절성 정동 장애 치료제로도 승인되었다.[175][176]

2008년 4월, FDA는 염산염 대신 브롬화수소산염 형태의 부프로피온 제제를 승인하여 사노피(Sanofi-Aventis)에서 아플렌진(Aplenzin)이라는 이름으로 판매하도록 했다.[181][182][183]

일본에서는 승인을 위한 임상 시험이 진행되었으나, 개발이 중단되었다.[214]

3. 약리 작용

부프로피온은 노르에피네프린-도파민 재흡수 억제제(NDRI)로, SSRI 계열 항우울제와는 다른 작용 기전을 가진다.[102] 노르에피네프린과 도파민의 재흡수를 억제하여 이들 신경전달물질의 농도를 높인다. 일반적인 항우울제에서 나타나는 위장 및 성기능 장애, 체중 증가 등의 부작용이 적다. 금연 시에는 도파민 재흡수를 억제하여 금단 증상을 완화하고 흡연 충동을 억제하는 효과가 있어 니코틴 의존 치료의 단기간 보조요법제로 사용된다.

부프로피온의 작용 기전은 명확하게 밝혀지지 않았지만,[102] 여러 니코틴성 아세틸콜린 수용체의 음성 알로스테릭 조절제로 작용한다는 사실과 관련이 있는 것으로 추정된다.[102] 부프로피온은 노르에피네프린-도파민 방출제는 아니다.[117]

부프로피온의 약리 작용은 활성 대사산물인 하이드록시부프로피온, ''트레오''-하이드로부프로피온, ''에리트로''-하이드로부프로피온에 의해 상당 부분 나타난다.[102] 이 대사산물들은 혈장 내에서 부프로피온과 비슷하거나 더 높은 농도로 존재하며, 특히 ''S,S''-하이드록시부프로피온은 노르에피네프린 및 도파민 재흡수 억제, 니코틴성 억제 작용을 보인다.[102] 부프로피온은 α-, β-아드레날린, 도파민, 세로토닌, 히스타민, 무스카린성 아세틸콜린 수용체 등 다양한 수용체에 대한 유의미한 직접적 활성은 없다.[31]

양전자 방출 단층 촬영술(PET) 연구에서 부프로피온(300mg/일)과 그 대사산물에 의한 도파민 수송체(DAT) 점유율은 약 20%(평균 14%~26%)로 나타났다.[118][119][120][47] 이는 치료 용량의 NDRI인 메틸페니데이트가 DAT 부위의 50% 이상을 점유하는 것에 비해 낮은 수치이다.[47] 낮은 DAT 점유율은 부프로피온의 약리 작용에서 도파민 재흡수 억제의 역할에 의문을 제기하며, 다른 작용이 치료 효과의 원인일 수 있음을 시사한다.[118][119][47][120]

부프로피온과 그 대사산물에 의한 노르에피네프린 수송체(NET) 점유율에 대한 자료는 없지만,[118][119] 하이드록시부프로피온의 노출 증가와 NET에 대한 더 높은 친화도를 고려하면,[145] 부프로피온의 전반적인 약리 작용은 도파민 재흡수 억제제보다 노르에피네프린 재흡수 억제제에 더 가까울 수 있다.[123][124] 따라서 부프로피온의 임상 효과는 도파민성 작용보다 노르아드레날린성 활성과 더 일치한다.[123][124]

3. 1. 작용 기전

부프로피온은 SSRI 계열 항우울제와는 달리 노르에피네프린-도파민 재흡수 억제제(NDRI)로 작용한다.[102] 이는 노르에피네프린과 도파민의 재흡수를 억제하여, 이 신경전달물질들의 농도를 높인다. 일반적인 항우울제 치료 시 흔하게 나타나는 위장 및 성기능 장애, 체중 증가 등의 부작용이 적다는 장점이 있다. 또한, 금연 기간 중 도파민 재흡수를 억제하여 금단 증상을 완화시켜 금연 보조제로도 사용된다.

부프로피온은 여러 니코틴성 아세틸콜린 수용체(nAChRs)의 음성 알로스테릭 조절제로 작용한다.[102] 설치류 대상 연구에서는 시그마-1 수용체(σ1) 작동제로 작용할 수 있다는 결과가 나왔다.[125][126][127]

3. 2. 약물 동태

경구 투여 후 부프로피온은 빠르게 흡수되어 1.5시간 후 최고 혈장 농도(tmax)에 도달한다. 서방형(SR) 및 서방형(XL) 제형은 흡수를 늦추도록 설계되어 각각 3시간 및 5시간의 tmax를 나타낸다.[99] 부프로피온의 절대적인 생체 이용률은 알려져 있지 않지만 초회 통과 효과로 인해 5~20%로 낮을 것으로 추정된다. XL 제형은 SR 제형 및 속방형 부프로피온에 비해 낮은 생체 이용률(68%)을 보인다.[102]

부프로피온은 체내에서 다양한 경로로 대사된다. 산화 경로는 시토크롬 P450 동종 효소 CYP2B6에 의해 ''R,R''-하이드록시부프로피온과 ''S,S''-하이드록시부프로피온을 생성하며, 더 적은 정도로 CYP2C19에 의해 4'-하이드록시부프로피온을 생성한다. 환원 경로는 간의 11β-하이드록시스테로이드 탈수소효소 1형 및 장의 AKR7A2/AKR7A3에 의해 ''트레오''-하이드로부프로피온을 생성하며, 아직 알려지지 않은 효소에 의해 ''에리트로''-하이드로부프로피온을 생성한다.[102]

부프로피온 대사는 매우 가변적이다. 동일한 양의 약물을 섭취한 사람에게서 부프로피온의 유효 용량은 최대 5.5배(생물학적 반감기 12~30시간)까지, 하이드록시부프로피온의 유효 용량은 최대 7.5배(반감기 15~25시간)까지 차이가 날 수 있다.[78][151][152] 일부 연구자들은 부프로피온과 하이드록시부프로피온의 혈중 농도 모니터링을 옹호하기도 한다.[153]

부프로피온 대사는 2상 약동학을 따르는 것으로 보인다. 약 1시간의 반감기를 갖는 재분배 알파 단계[154]는 약 12-30시간의 대사 베타 단계에 선행한다. 이는 짧은 "흥분"으로 인해 남용이 불가능하며, 부프로피온의 일관된 농도를 유지하기 위해 서방형 제제를 사용하는 이유를 설명할 수 있다.

부프로피온 대사는 종에 따라 크게 다르다.[155][156][157] 예를 들어, 경구 부프로피온은 사람에서 부프로피온 자체보다 하이드록시부프로피온의 수준이 16배 더 높게 나타나는 반면, 쥐에서는 경구 부프로피온이 하이드록시부프로피온보다 부프로피온의 수준이 3.4배 더 높게 나타난다.[155] 부프로피온의 종별 대사는 약력학적 효과의 종별 차이에 관여하는 것으로 생각된다.[155][156][157] 예를 들어, 부프로피온은 설치류에서 정신 자극과 유사한 효과 및 강화 효과를 나타내는 반면, 치료 용량의 경구 부프로피온은 인간에게서 이러한 효과를 거의 또는 전혀 나타내지 않는 것으로 보인다.[158]

4. 임상적 사용

부프로피온은 우울증 치료에 효과적인 항우울제이다. 선택적 세로토닌 재흡수 억제제(SSRIs)와 같은 다른 항우울제와 비교했을 때, 성기능 장애를 일으킬 가능성이 적고 체중 증가와 졸음을 유발하지 않는다는 특징이 있다.[21][30][31] 오히려 체중 감소를 유도하고, 과다 수면 및 피로 증상을 개선하는 데 SSRIs보다 효과적이다.[21][32] SSRIs에 반응하지 않는 환자에게 부프로피온을 추가하는 것은 임상적으로 효과가 있는 전략이다.[35]

부프로피온은 금연 치료에도 사용된다. 흡연 욕구와 금단 증상을 줄여주어 금연 성공률을 높이는 데 도움을 준다.[37][38][39][40] 금연 치료 효과는 시간이 지남에 따라 감소하지만, 위약에 비해 금연 유지에 효과적이다.[41][43]

주의력결핍 과잉행동장애(ADHD) 치료에도 사용될 수 있지만, 이에 대한 연구는 제한적이다.[46][47][48][49][50]

4. 1. 우울증

부프로피온은 우울증 치료에 있어 전반적으로 위약보다 효과적이며, 다른 항우울제와 유사한 효과를 보인다.[20][21][22][23][24] 가을과 겨울철 동안 부프로피온은 재발성 계절성 정동 장애가 있는 사람들에게 우울증 발병을 예방하는데, 부프로피온 투여 환자의 15%가 주요 우울 에피소드를 경험한 반면, 위약 투여 환자는 27%였다.[28] 또한 양극성 장애에서 우울증을 개선하며, 효과와 정동 전환 위험은 다른 항우울제와 유사하다.[29]

부프로피온은 다른 항우울제와 구별되는 몇 가지 특징이 있다. 대부분의 항우울제와 달리 일반적으로 성기능 장애를 일으키지 않으며, 성적 부작용 발생은 위약과 다르지 않다.[21][30] 부프로피온 치료는 체중 증가와 관련이 없으며, 오히려 대부분의 연구에서 상당한 체중 감소가 관찰되었다.[21] 또한 다른 항우울제가 유발할 수 있는 기면과 관련이 없으며,[31] 우울증 환자의 과다 수면 및 피로 증상을 개선하는 데 선택적 세로토닌 재흡수 억제제(SSRIs)보다 효과적이다.[32]

SSRI에 반응하지 않는 사람들에게 부프로피온을 처방된 SSRI에 추가하는 것은 흔한 전략이며, 임상 시험에서 뒷받침된다.[35]

4. 2. 금연

부프로피온은 노르에피네프린-도파민 재흡수 억제제(NDRI) 작용을 통해 금연 시 흡연 충동을 억제하는 니코틴 의존 치료의 단기간 보조 요법제로 이용된다.[37][38][39][40] 부프로피온은 니코틴에 대한 갈망과 우울한 기분, 과민성, 집중력 저하, 식욕 증가와 같은 금단 증상의 심각성을 줄여준다.[40] 또한 금연 후 처음 몇 주 동안 종종 발생하는 체중 증가를 늦추지만, 시간이 지남에 따라 이러한 효과는 미미해진다.

부프로피온 치료 과정은 보통 7~12주 동안 지속되며, 환자는 치료 시작 후 10일 정도 후에 담배 사용을 중단한다.[40][78] 치료 후 금연 유지 효과는 시간이 지나면서 감소하여 3개월 시점에 37%, 1년 시점에 20%로 떨어진다.[41] 부프로피온 치료 기간을 연장하는 것이 흡연 재발을 방지하는 데 도움이 되는지는 불분명하다.[42]

치료 후 6개월 시점에서 부프로피온은 위약에 비해 흡연 중단 가능성을 약 1.6배 증가시킨다. 이는 니코틴 대체 요법만큼 효과적이지만 바레니클린보다는 효과가 떨어진다. 부프로피온과 니코틴 대체 요법을 병용해도 금연율이 향상되지 않는다.[43]

어린이와 청소년의 경우, 흡연 중단을 위해 부프로피온을 사용하는 것은 유의미한 이점을 제공하지 않는 것으로 보인다.[44] 임산부의 흡연 중단을 돕기 위한 사용에 대한 증거는 불충분하다.[45]

미국 소아과 학회의 2019년 지침에는 주의력결핍 과잉행동장애(ADHD) 치료에 부프로피온이 승인된 적응증이 아니라고 언급되어 있다.[46] 성인과 아동 모두에서 ADHD 치료를 위한 부프로피온에 대한 체계적 검토에서는 효과적일 수 있다고 언급하지만, 임상 시험의 규모가 작고 편향의 위험이 있어 신중하게 해석해야 한다고 경고한다.[47][48][49][50] 아토목세틴과 마찬가지로, 부프로피온은 ADHD에 대해 작용 시작이 지연되어 치료 효과를 얻으려면 수 주간의 치료가 필요하다.[47][51] 이는 즉각적인 효과를 보이는 암페타민 및 메틸페니데이트와 같은 자극제와 대조적이다.[51]

5. 부작용 및 주의사항

글락소스미스클라인에서는 간질 환자, 발작 역치를 낮출 수 있는 "알코올 및 벤조디아제핀을 포함한 항불안제의 갑작스러운 복용 중단을 하고 있는 환자", 거식증 환자, 폭식증 환자, 뇌종양 환자에게 부프로피온을 처방해서는 안 된다고 밝히고 있다.[216] 미국 식품의약국(FDA) 첨부 문서에서는 모노아민 산화 효소 억제제(MAOI)를 복용하고 있는 환자에 대한 처방도 피하고, MAOI에서 부프로피온으로 처방을 변경할 경우에는 최소 2주 간격을 두어야 한다고 명시하고 있다. 또한, 자살 충동 위험이 증가하므로 간 장애, 심각한 신장 질환, 심각한 고혈압을 가진 환자, 소아, 사춘기, 청소년, 청년 성인에게는 충분히 주의하여 처방해야 한다.[216]

1993년 증례 보고에서는 부프로피온이 주의력 결핍 과잉 행동 장애에 뚜렛 증후군을 병발하고 있는 소아에게서 틱을 악화시킬 수 있다고 보고되었다.[217]

5. 1. 부작용

부프로피온은 다른 항우울제에 비해 성 기능 장애를 일으킬 가능성이 적다.[52] 여러 연구에 따르면 부프로피온은 다른 항우울제보다 성적 부작용이 적을 뿐만 아니라, 선택적 세로토닌 재흡수 억제제(SSRI) 항우울제로 인해 발생한 성 기능 장애를 완화하는 데 도움이 될 수 있다.[53][54] 또한, 부프로피온 또는 부프로피온과 트라조돈을 함께 복용하는 것이 성욕 저하 장애(HSDD)가 있고 우울증이 없는 여성의 일부 성 기능 지표를 개선할 수 있다는 소규모 연구도 있다.[55]

일반적인 부작용으로는 구강 건조, 메스꺼움, 변비, 불면증, 불안, 떨림, 과도한 발한 등이 있다.[78][7] 부프로피온은 데스벤라팍신을 제외한 모든 2세대 항우울제 중 불면증 발생률이 가장 높다.[79] 또한 두통 위험이 약 20% 증가하는 것과 관련이 있다.[80]

부프로피온은 일부 사람들의 혈압을 상승시킨다. 한 연구에 따르면 환자의 10%에서 수축기 혈압이 평균 6 mm Hg 상승했다.[81] 부프로피온을 복용하는 일부 사람들에게는 기존 고혈압 유무와 관계없이 때때로 심각한 고혈압이 관찰된다.[78][7]

발작은 부프로피온의 드물지만 심각한 부작용이며, 용량 의존성이 강하다. 속방형 제제의 경우, 하루 300–450 mg 용량에서 발작 발생률은 0.4%이며, 권장 용량보다 높은 600 mg에서는 발생률이 거의 10배 증가한다.[78][7] 일반 인구에서 유발되지 않은 발작의 발생률은 0.07–0.09%이며, 다른 다양한 항우울제의 발작 위험은 권장 용량에서 일반적으로 0–0.5%이다.[84]

부프로피온에 대해 사망 또는 간 이식으로 이어지는 간 독성 사례가 보고되었다.[85] 처방 정보는 부프로피온이 급성 폐쇄각 녹내장 발작을 유발할 수 있음을 경고한다.[78] 반면, 부프로피온은 개방각 녹내장 발생 위험을 감소시킬 수 있다.[86]

임신 첫 삼 분기에 부프로피온을 사용하는 어머니는 자녀의 선천성 심장 결손 위험이 23% 증가하는 것과 관련이 있다.[87] 드물게 스티븐스-존슨 증후군 사례와 관련이 있다.[88][89]

드물게 부프로피온 유발 정신병이 발생할 수 있다. 이는 부프로피온의 더 높은 용량과 관련이 있으며, 많은 사례가 권장 용량보다 높은 용량에서 보고된다. 동시 투여하는 항정신병 약물이 보호 효과를 보이는 것으로 보인다.[94] 대부분의 경우 정신병 증상은 용량을 줄이거나, 치료를 중단하거나, 항정신병 약물을 추가하여 제거된다.[78][94]

연구가 부족하지만, 소수의 증례 보고에 따르면 부프로피온의 갑작스러운 중단이 항우울제 중단 증후군을 유발할 수 있다.[95]

5. 2. 주의사항
