1. 개요

2. 적응증

파록세틴은 우울증, 강박 장애, 외상후 스트레스 장애, 공황 장애, 범불안 장애,[192] 사회 공포증,[193] 월경전 불쾌장애에 사용되는 항우울제이다.[194] 미국에서는 공황 발작 치료에 처음 사용되는 항우울제이기도 하다.[195]

주로 주요 우울 장애, 강박 장애, 외상 후 스트레스 장애, 사회 불안 장애, 공황 장애 치료에 사용되며, 광장 공포증, 범불안 장애, 월경 전 불쾌 장애, 갱년기 안면 홍조에도 가끔 사용된다.[17][18][19][20][21]

공황 장애 치료를 위해 미국에서 승인된 최초의 항우울제였다.[26] 여러 연구에서 파록세틴이 공황 장애 치료에 위약보다 효과적이라는 결론을 내렸다.[24][27]

성인 및 아동의 사회 불안 장애 치료에 효과가 있으며,[28][29] 사회 불안 장애와 알코올 사용 장애를 함께 겪는 사람들에게도 도움이 된다.[30] 다른 여러 SSRI와 유사한 것으로 보인다.[31]

3. 효능 및 효과

파록세틴은 우울증, 강박 장애, 외상후 스트레스 장애, 공황 장애, 범불안장애,[192] 사회 공포증,[193] 월경전 불쾌장애[194] 등에 사용되는 항우울제이다.

미국에서는 공황 발작 치료에 쓰이는 항우울제이기도 하다.[195] 글락소스미스클라인과 미국 식품의약국의 허가 내용에 따르면, 파록세틴은 우울증의 경우 위약보다 6배, 공황 장애에 대해서는 3번에 걸친 10~12주간의 연구 결과 위약보다 뛰어난 효능을 보였다.[200] 다른 연구에서는 환자들의 10~20%가 더 많은 파록세틴의 효능을 봤다고 한다.[196]

주로 주요 우울 장애, 강박 장애, 외상 후 스트레스 장애, 사회 불안 장애, 공황 장애를 치료하는 데 사용된다. 광장 공포증, 범불안 장애, 월경 전 불쾌 장애, 갱년기 안면 홍조에도 가끔 사용된다.[17][18][19][20][21]

여러 메타 분석 결과, 파록세틴은 위약보다 우수하거나 다른 항우울제와 동등한 효과를 보였다.[22][23][24] 그러나 다른 항우울제에 비해 치료 반응이 더 낫거나 나쁘다는 명확한 증거는 없었다.[25]

3. 1. 논란

다른 항우울제와 마찬가지로, 파록세틴은 25세 미만의 사람들에게 자살 사고 및 행동의 위험을 증가시킬 수 있다.[48][49] 2004년 FDA는 파록세틴의 어린이 및 청소년 임상 시험에 대한 통계 분석을 수행했으며, 우울증과 불안 장애 모두에 대한 시험에서 위약에 비해 자살 및 자살 관념이 증가한 것을 발견했다.[50] 2015년 ''영국 의학 저널''에 게재된 논문은 파록세틴과 이미프라민을 우울증을 앓는 청소년을 대상으로 위약과 비교 평가한 연구 329에서[51] 자살 행동의 발생이 과소 보고되었고 파록세틴의 효능이 과장되었다고 주장하며 원래의 사례 노트를 재분석했다.[52][53][54][55][56]

1998년 공개된 문헌을 메타 분석한 결과, 파록세틴을 포함한 16종의 항우울제 및 정신과 약물은 대체로 우울증에 대한 약물의 본래 효과는 25%이고 나머지는 위약 효과나 자연 경과임이 나타났다.[143]

2004년 글락소스미스클라인에 의한 시험 정보 은폐를 위한 재판 결과, 파록실에 관한 모든 임상 시험 정보를 인터넷으로 공개하도록 요구받았다.[144] 미공개 데이터가 나오면서 출판 편향을 시정하여 데이터를 분석하는 것이 가능하게 되었다.

2008년 세계 보건 기구와 그 관련 기관이, 제재에 의해 공개된 파록세틴에 관한 미공개 시험을 결합하여 40개 시험의 총 3704명으로 구성된 이중 맹검 시험을 분석한 결과, 파록세틴의 본래 효과는 효과 전체의 17%이며, 나머지는 위약 효과나 자연 경과임이 밝혀졌다.[145] 이는 SSRI인 파록세틴과 플루옥세틴, SNRI인 벤라팍신, 다른 기전의 항우울제 nefazodone|네파조돈영어의 미공개 문헌을 포함하여 분석하여 밝혀진, 약물의 본래 효과는 18%라는 연구와 거의 일치한다.[146]

사춘기(12~18세)의 우울증 환자를 대상으로 한 2001년의 임상 시험이 재분석된 결과, 파록세틴 투여군과 위약군 간의 해밀턴 우울증 평가 척도(HAM-D) 점수 변화는 통계적으로나 임상적으로 유의미한 차이를 보이지 않았다.[134] 이 시험에서 자살·자해 행동은 위약군에서 2.3%(2/87), 파록세틴군에서 11.8%(11/93)로 나타나 파록세틴 투여군에서 더 높은 발생률을 보였다.

4. 부작용

파록세틴의 일반적인 부작용으로는 구역질, 졸음, 구강 건조, 어지럼증, 변비, 두통, 식욕 부진, 성 기능 장애 등이 있다. 이러한 부작용은 대부분 일시적이며, 지속적인 치료를 받으면 사라진다.[1] 하지만, 몸이 약에 적응하는 처음 1~4주 동안 부작용이 나타날 수 있으며, 약의 양에 따라 증상이 호전되거나 악화될 수 있다.[200]

드물지만 중대한 부작용으로는 세로토닌 증후군(혼란, 발열, 발한, 떨림, 경련 등), 악성 증후군(몸의 심한 경직, 의식 불명, 고열 등), 간 기능 장애(황달), 조증 등이 나타날 수 있다.[6] 특히 25세 미만의 젊은층에게서는 자살 충동을 일으킬 수 있어 주의가 필요하다.[6] 미국 식품의약국(FDA)은 파록세틴에 자살 위험에 대한 블랙박스 경고를 부착하도록 하였다.[197]

파록세틴은 남성과 여성 모두에게 성 기능 장애를 일으킬 수 있다.[210] 남성의 경우 정자의 DNA를 손상시킬 수 있으며,[57] 성욕 감퇴, 무쾌감증, 발기 부전 등의 부작용이 나타날 수 있다.

이 외에도 안절부절못함, 과활동 증상,[204][205][206][207] 기억력 감퇴[215]등이 보고되었다. 또한, 알코올과 함께 복용하면 부작용이 더 쉽게 나타날 수 있으므로 주의해야 한다.

4. 1. 금단 증후군 (SSRI 중단 증후군)

파록세틴은 다른 선택적 세로토닌 재흡수 억제제(SSRI) 계열 약물보다 금단 증후군 발생률과 심각도가 높은 것으로 알려져 있다.[71][147]

일반적인 금단 증상으로는 메스꺼움, 어지럼증, 현기증, 수면 장애, 악몽 및 생생한 꿈, 신체의 전기가 흐르는 듯한 느낌, 반동 우울증, 불안 등이 있다.[72][148][149]

금단 증상을 예방하기 위해 파록세틴 액체 제형을 사용하여 용량을 매우 점진적으로 줄일 수 있다. 또는 반감기가 더 길어 중단 증후군의 심각성을 줄여주는 플루옥세틴으로 일시적으로 전환하는 방법도 권장된다.[72][73][74][137][150][151]

2002년, 미국 FDA는 파록세틴 치료를 중단하는 사람들에게서 감각 이상, 악몽, 어지럼증을 포함한 "심각한" 중단 증상에 대한 경고를 발표했다. 또한, 초조함, 발한, 메스꺼움에 대한 사례 보고서도 경고했다. 국제 제약 협회 연맹은 금단 반응이 환자의 0.2%에서만 발생하며 "경미하고 짧다"는 글락소스미스클라인 대변인의 발언과 관련하여 GSK가 연맹의 실천 강령 중 두 가지를 위반했다고 밝혔다.[75] 글락소스미스클라인에 게시된 파록세틴 처방 정보는 심각한 중단 증상을 포함한 중단 증후군의 발생과 관련하여 업데이트되었다.[65] 유럽 의약품청의 의약품 사용 위원회는 중단을 결정하는 경우 수 주 또는 수 개월에 걸쳐 점진적으로 감량할 것을 권장한다.[43]

4. 2. 임신 및 수유

임신 중 파록세틴 사용은 선천적 결손, 특히 심장 기형의 위험 증가와 관련이 있으므로, 미국 산부인과 학회는 임신 중이거나 임신을 계획 중인 여성의 경우 "가능하면 피해야 한다"고 권고한다.[63][64] 임신 첫 삼 분기에 파록세틴을 사용한 여성에게서 태어난 아기는 심혈관 기형, 주로 심실 및 심방 중격 결손의 위험이 증가한다.[65] 따라서 파록세틴의 이점이 치료를 계속할 이유가 되지 않는 한, 다른 항우울제로 중단하거나 변경하는 것을 고려해야 한다. 임신 중 파록세틴 사용은 선천적 결손(특히 심장 결함, 구순구개열, 굽은 발)이 약 1.5~1.7배 증가하는 것과 관련이 있다.[66][67][68][69][70] 항우울제 노출(파록세틴 포함)은 임신 기간 단축(3일), 조산 위험 증가(55%), 저체중아 출산 위험 증가(75g), 낮은 아프가 점수(<0.4점)와 관련이 있다.[61][62]

모유 수유 중에는 파록세틴 사용이 비교적 안전한 것으로 알려져 있다.[10]

5. 약물 상호 작용

글락소스미스클라인은 파록세틴과의 약물 상호 작용이 신경이완제악성증후군과 같은 특별한 문제를 일으키거나 증상을 악화시킬 수 있다고 경고한다. 잠재적인 세로토닌 증후군이나 신경이완제악성증후군과 유사한 반응은 SNRI 및 SSRI 단독 투여 시에도 나타날 수 있으며, 파록세틴도 이에 포함된다. 그러나 이러한 반응은 세로토닌성 약물(트립토판 포함), 세로토닌 대사를 방해하는 약물(MAOI 포함), 항정신병 약물 또는 다른 도파민 길항제와 함께 복용할 때 더 흔하게 나타난다.[216]

파록세틴은 MAOI 계열 항우울제(선택적이지 않은 MAOI의 반대 작용을 하는 항생제 리네졸리드 포함)와 함께 처방하거나, MAOI 복용 중단 후 14일 이내에 복용해서는 안 된다. 또한 피모자이드, 티오리다진, 트립토판, 와파린과 함께 투여하는 것도 피해야 한다.[216]

유방암 환자가 유방암 치료제인 타목시펜과 함께 파록세틴을 복용하면 타목시펜의 효능이 감소하여 유방암으로 인한 사망률이 24~91%까지 증가한다는 연구 결과도 있다.[217][218] 세로토닌 계통에 작용하거나 세로토닌 대사를 손상시키는 다른 약물과 파록세틴을 함께 복용하면 세로토닌 증후군 또는 신경이완제 악성 증후군 (NMS) 유사 반응의 위험이 커진다. 이러한 반응은 SNRI 및 SSRI 단독으로도 나타날 수 있지만, 트립탄, MAO 억제제, 항정신병 약물, 기타 도파민 길항제를 함께 사용할 때 특히 주의해야 한다.

파록세틴 처방 정보에는 "MAOI (가역적 비선택적 MAOI인 항생제 리네졸리드 포함)와 함께 사용하거나 MAOI 치료 중단 후 14일 이내에 사용해서는 안 된다"고 명시되어 있으며, 피모자이드, 티오리다진, 트립토판, 와파린과 함께 사용해서는 안 된다는 내용이 포함되어 있다.[65]

6. 금기

다음과 같은 경우에는 파록세틴을 투여해서는 안 된다.


  • 어린이 및 청소년. NICE의 가이드라인에서는 파록세틴 및 벤라팍신은 어린이 및 청소년의 우울증 치료로 사용해서는 안 된다고 권고하고 있다.[152] NICE가 18세 미만에 허가한 항우울제는 플루옥세틴 단 하나뿐이며, 심각한 우울증에 한정되며, 또한 심리 요법을 병용해야 한다.[135]
  • 파록세틴에 대해 과거에 과민증을 나타낸 환자.
  • 아토목세틴 복용자[153]
  • MAO 억제제를 복용 중이거나, 복용을 중단한 지 2주가 안 된 환자.
  • 뇌내 세로토닌 농도가 높아지기 때문이다.
  • 염산 치오리다진(상품명: 메레릴) 투여 중인 환자.
  • 파록세틴이 약물 대사 효소의 작용을 방해하여, 치오리다진의 혈중 농도가 상승할 위험이 있기 때문이다.
  • 심실성 부정맥이나, 심전도상 QT 간격이 연장되는 등, 심장의 움직임과 관련된 심각한 부작용을 동반하기 때문이다.
  • 피모지드(상품명: 오라프)를 복용 중인 환자.
  • 치오리다진의 경우와 동일.


또한, 파록세틴 복용 중에는 세인트 존스 워트를 포함한 식품을 섭취하지 않도록 하는 것도 중요하다. 섭취하면 약의 효용을 높여 버릴 위험이 있다.

7. 약리학

파록세틴은 가장 강력하고 특이성이 높은 선택적 세로토닌 재흡수 억제제(SSRI) 중 하나이다.[82] 세로토닌 수송체(SERT)에 결합하여 세로토닌 재흡수를 억제하고, 시냅스 간극의 세로토닌 농도를 증가시킨다. 에스시탈로프람과 유사하게 세로토닌 수송체의 알로스테릭 조절 부위에 결합하지만, 그 효능은 덜하다.[83] 노르에피네프린 수송체(NET)에도 약한 억제 효과를 나타낼 수 있다(<50 nmol/L).[84] 쥐에게 4주 동안 투여한 증거에 따르면, 파록세틴 20 mg을 1일 1회 복용하는 것은 전전두피질에서 세로토닌 수송체의 약 88%를 점유한다.[80] 파록세틴은 페닐피페리딘이며 오피오이드 수용체에 어느 정도 친화성을 가질 수 있다.[85]

파록세틴은 무스카린성 아세틸콜린 수용체(M1)에 대한 친화성이 있어 항콜린성 부작용(구강 건조, 변비 등)을 유발할 수 있다.

파록세틴은 경구 투여 후 잘 흡수된다.[80] 절대 생체이용률은 약 50%이며, 포화 가능한 초회 통과 효과가 있다는 증거가 있다.[89] 경구 복용 시 약 6~10시간 내에 최고 농도에 도달하며[80] 7~14일 내에 정상 상태에 도달한다.[89] 파록세틴은 분포 용적과 청소율에서 개체 간에 상당한 차이를 보인다.[89] 경구 투여량의 2% 미만이 변화 없이 소변으로 배설된다.[89]

8. 사회 문화적 측면

일본에서는 5mg, 10mg, 20mg 정제와 6.25mg, 12.5mg, 25mg 장용성 제제가 사용되며, 여러 회사에서 제네릭 의약품도 판매하고 있다. 일본 산도 제약 주식회사에서 제조하는 파록세틴 정 10mg 제네릭 의약품은 어린이가 잘못 복용하지 않도록 PTP 포장에서 꺼내기 어렵게 되어 있다.

2012년 1월 19일, GSK는 1월 18일부로 서방성 제제인 「팍실 CR (Controlled Release)」정의 제조 판매 승인을 취득했다고 발표했다.[154] 같은 해 6월 22일부 관보에는 팍실 CR 정 및 팍실(파록세틴)의 제네릭 의약품이 약가 수재되었다.

8. 1. 마케팅 및 소송

글락소스미스클라인(GSK)은 파록세틴을 마케팅하면서 어린이 대상 허가 외 사용, 부정적인 연구 결과 은폐, 금단 증상 미고지 등의 문제로 인해 막대한 벌금을 물고 소송에 휘말렸다.[94][95]

2004년, GSK는 소비자 기만 혐의로 250만 USD에 합의했다.[92] 법적 증거개시 과정에서 GSK 내부 문건에 "어린이에게 효과가 없다는 내용을 포함하면 파록세틴의 평판이 나빠질 것이다"라고 적혀 있는 등, 불리한 연구 결과를 의도적이고 체계적으로 숨긴 증거가 드러났다.[93]

2012년, 미국 법무부는 GSK에게 데이터 은폐, 18세 미만 환자에게 불법 권장, 연구 329 관련 기사 오도 등의 혐의로 30억 USD의 벌금을 부과했다.[94][95][96]

2016년 2월, 영국 경쟁 및 시장청은 유럽 연합 및 영국 경쟁법 위반, 파록세틴 복제약 출시 지연 합의 혐의로 GSK에 37,600,757GBP, 기타 복제약 회사들에 총 7,384,146GBP의 벌금을 부과했다. 영국 공공 보건 서비스는 복제약이 불법적으로 차단된 기간 동안의 손해 보상을 청구할 가능성이 높으며, GSK는 반경쟁적 행위로 손실을 입은 다른 복제약 제조업체로부터 소송을 당할 수도 있다.[97] 2016년 4월, 벌금 부과 회사들은 경쟁 항소 법원에 항소했다.[98][99][100][101][102]

2001년 9.11 테러 이후, GSK는 미국에서 팍실 TV 광고를 늘렸고, 2001년 10월에는 2000년 10월의 거의 2배에 달하는 비용을 썼다.[162] 파록세틴 중단 어려움과 GSK의 은폐는 아메리칸 브로드캐스팅 컴퍼니(ABC)에서 보도되었다.[163]

영국에서는 2001년 이후 세록사트(영국 상표명) 처방자를 대표하여 소송이 제기되었다. 이들은 GSK가 환자 정보를 무시하고 약물에 심각한 부작용이 있다고 주장했다.[164][165]

2004년 초, GSK는 소비자 사기 소송을 2500000USD에 합의했다(당시 연간 270억달러가 넘는 팍실 매출의 일부).[166] 법적 증거 개시 절차에서 GSK 내부 문서에 "[소아에서의] 유효성이 증명되지 않았다는 기재를 포함하면 파록세틴의 평판을 훼손할 것이다"라고 쓰여 있었다.[167]

2004년 6월, FDA는 팍실 CR의 "허위 또는 과장된" TV 광고에 대해 GSK에 위반 문서를 공개했다.[168] GSK는 광고가 FDA 심사를 거쳤지만, 다시 방영하지 않겠다고 밝혔다.[169]

2007년 1월 29일, BBC "파노라마"는 세록사트에 관한 다큐멘터리를 방영했다.[170] 어린이와 청소년 대상 3건의 GSK 소아 임상 시험 결과, 10대에게는 효과가 없고, 자살 위험이 6배 높았다. 연구 329는 파록세틴 안전성과 유효성을 오도하는 방식으로 보고되었다.[171][172]

2008년 10월, 법원 문서는 GSK와 연구자가 임상 시험 동안 자살 위험 데이터를 억제하거나 숨겼을 가능성을 보여준다. 찰스 네메로프는 2006년 GSK로부터 960,000USD 이상을 받았지만 35,000USD만 밝혔고, 2000년부터 2006년 사이 제약 회사 지불액이 총 2,500,000USD 이상이었다.[173]

9. 기타 연구

여러 연구에서 파록세틴이 조루증 치료에 사용될 수 있음을 시사했다. 특히 질내 사정 잠복기(IELT)가 6~13배 증가했는데, 이는 다른 SSRI(플루복사민, 플루옥세틴, 설트랄린, 시탈로프람) 치료로 얻은 지연보다 다소 길었다.[110][111][112] 그러나 성관계 3~10시간 전에 급성으로 ("요구 시") 파록세틴을 복용하면 사정이 "임상적으로 무의미하고 성적으로 불만족스러운" 1.5배 지연만 발생했으며, 4배 지연을 유도하는 클로미프라민보다 열등했다.[112]

파록세틴이 강박적 도박[113] 및 열감 치료에도 효과적일 수 있다는 증거도 있다.[114]

당뇨병성 신경병증[115] 또는 만성 긴장성 두통[116]에 대한 파록세틴 처방의 이점은 불확실하다.

증거가 상충되기는 하지만, 파록세틴은 일년 중 대부분의 날에 우울 증상을 동반하는 만성 질환인 기분부전증 치료에 효과적일 수 있다.[117]

파록세틴이 심부전이 있는 쥐에서 G 단백질 연결 수용체 키나아제 2 (GRK2)에 선택적으로 결합하여 억제한다는 것을 뒷받침하는 증거가 있다. GRK2는 심부전의 경우 둔감해지는 베타 아드레날린 수용체의 활성을 조절하므로, 파록세틴(또는 파록세틴 유도체)은 미래에 심부전 치료제로 사용될 수 있다.[118][119][120]

파록세틴은 잠재적인 질병 완화 골관절염 약물로 확인되었다.[121]

파록세틴은 폐수에서 흔히 발견된다.[123] 이는 조류인 ''Pseudokirchneriella subcapitata''(동의어: ''Raphidocelis subcapitata'')에 매우 유독하다.[123] 또한 토양 선충인 ''Caenorhabditis elegans''에도 독성이 있다.[124]

Alberca 등(2016)은 파록세틴이 ''T. cruzi''에 대한 트리파노사이드로 작용한다는 것을 발견했다.[125] Alberca 등(2016)은 리슈만충 살충제 효과를 발견했다.[126] Alberca는 파록세틴이 ''L. infantum''의 전(前)원충에 세포 사멸을 일으킨다는 것을 발견했다.[126] 작용 기전은 알려져 있지 않다.[126]
