2. 적응증

벤라팍신은 우울증, 범불안 장애, 사회 공포증, 공황 장애 치료에 사용된다.[11]

벤라팍신은 허가 외로 당뇨병성 신경병증[12] 및 편두통 예방[13] 치료에도 사용되어 왔다. 이는 오피오이드 수용체에 미치는 영향을 통해 통증에 작용할 수 있다.[14] 또한 폐경기 여성 및 전립선암 치료를 위한 호르몬 요법을 받는 남성의 '열성 홍조'를 줄이는 것으로 나타났다.[15][16]

2. 1. 주요 우울 장애

21가지 주요 항우울제를 비교 분석한 메타 분석 결과, 벤라팍신은 아고멜라틴, 아미트리프틸린, 에스시탈로프람, 미르타자핀, 파록세틴, 보티옥세틴 등 다른 항우울제보다 효과가 더 좋았다. 그러나 이러한 비교 연구들의 질은 낮거나 매우 낮게 평가되었다.[21][22]

벤라팍신은 비정형 항우울제인 부프로피온과 효능이 비슷했지만, 증상이 완전히 사라지는 관해율은 벤라팍신이 더 낮았다.[23] SSRI에 반응하지 않는 환자들을 대상으로 한 이중 맹검 연구에서는 벤라팍신이나 다른 SSRI (시탈로프람)로 약물을 변경했을 때 두 그룹 모두 비슷한 수준으로 개선되었다.[24]

벤라팍신은 고용량(하루 225mg 및 375mg)이 저용량(하루 75mg)보다 효과가 더 좋지만, 부작용도 더 많이 나타난다.[32] 연구에 따르면 벤라팍신의 서방형이 속방형보다 우수하다.[36] 2017년 메타 분석에서는 벤라팍신의 효능이 우울증의 기저 심각도와 상관관계가 없다는 결과가 나왔다. 즉, 치료 시작 시점의 우울증 심각도와 관계없이 벤라팍신의 효능은 일관적으로 유지된다.[36]

일본에서 벤라팍신은 "우울증·우울 상태"에 효과가 있다고 승인되었다.

여러 이중 맹검 시험에서 벤라팍신이 우울증에 효과가 있다는 것이 밝혀졌다. 벤라팍신은 삼환계 항우울제인 아미트립틸린이나 이미프라민과 효과가 비슷하고, 아미트립틸린보다 내약성이 더 좋았다. 시험에서 사용된 평가 척도를 기준으로 보면 벤라팍신의 유효성은 설트랄린이나 플루옥세틴과 같거나 그 이상이었다. 고용량 벤라팍신을 사용한 경우 관해가 유지되거나 "매우 개선"된 환자가 많았지만, "반응" 또는 "개선"된 환자 수는 비슷했다. 벤라팍신과 SSRI 및 삼환계 항우울제를 메타 분석으로 비교했을 때 벤라팍신이 가장 우수했다.[120] 같은 기준으로 평가했을 때 벤라팍신은 비정형 항우울제 부프로피온과 효과가 비슷했지만, 관해(치유)율은 벤라팍신이 더 낮았다.[121] SSRI에 반응하지 않은 환자를 벤라팍신이나 시탈로프람으로 전환한 이중 맹검 시험에서는 양쪽 그룹 모두 비슷한 개선율을 보였다.[122]

항정신병약인 아리피프라졸과 함께 사용하면 고령자의 난치성 우울증 치료 성적이 향상되는 것으로 나타났다.[123] 그러나 고령자에게 아리피프라졸을 포함한 항정신병약을 사용한 시험에서는 부작용으로 대사 마커(BMI, 혈압, 혈당, 콜레스테롤, 트리글리세라이드)가 악화되고 36.5%가 대사 증후군을 겪는 등 위험성/이익 비율이 좋지 않으므로 충분한 상담이 필요하다.[124]

3. 약리작용

벤라팍신은 세로토닌-노르에피네프린 재흡수 억제제(SNRI)이지만, 세로토닌-노르에피네프린-도파민 재흡수 억제제(SNDRI)로도 언급된다.[72][73] 이 약물은 기분에 영향을 미치는 주요 신경전달물질인 세로토닌, 노르에피네프린, 그리고 고용량에서 도파민의 재흡수를 억제하여 시냅스 내에 더 많은 활성 신경전달물질을 남김으로써 작용한다.[76] 또한, 벤라팍신은 오피오이드 수용체와 α2-아드레날린 수용체에도 간접적으로 영향을 미쳐 쥐 실험에서 통증 역치를 증가시키는 것으로 나타났다.[80][14]

3. 1. 세로토닌-노르에피네프린 재흡수 억제 (SNRI)

벤라팍신은 세로토닌-노르에피네프린 재흡수 억제제(SNRI)로 분류되지만, serotonin-norepinephrine-dopamine reuptake inhibitor|세로토닌-노르에피네프린-도파민 재흡수 억제제영어(SNDRI)로 여겨지는 경우도 있다.[156][157] 기분에 영향을 미치는 주요 신경 전달 물질의 Monoamine transporter|모노아민 수송체|label=재흡수 단백질영어를 억제하여 시냅스에 남아있는 활성 신경 전달 물질을 증가시킨다. 영향을 받는 신경 전달 물질은 세로토닌, 노르에피네프린이다. 고용량에서는 도파민의 재흡수를 약하게 억제한다.[158] 전두엽에서는 도파민이 norepinephrine transporter|노르에피네프린 수송체영어에 의해 재흡수되어 비활성화되는데, 전두엽 대부분에는 도파민 수송체가 없기 때문에 벤라팍신이 이 부분에서 도파민의 신경 전달을 증가시킬 수 있다.[159][160]

벤라팍신은 저용량(150mg/일 미만)에서는 세로토닌 작용성 신경에만 작용하지만, 중등량(150mg/일 이상 300mg/일 이하)에서는 세로토닌 및 노르아드레날린 작용계에도 작용한다. 고용량(300mg/일 초과)에서는 도파민 작용성 전달에도 영향을 미친다.[112]

벤라팍신은 간접적으로 오피오이드 수용체(μ-, κ1-, κ3-, δ-서브타입)와 α1 아드레날린 수용체에도 작용하여 마우스에서 통증 역치를 상승시킨다. 마우스에 약물을 복강 내 투여하고 핫 플레이트 진통계로 통증을 측정하면, 벤라팍신과 미르타자핀 모두 용량 의존적으로 날록손에 의해 가역적인 항-침해 수용 작용을 나타냈다. 이는 벤라팍신이 마약이 필요한 중증 우울증의 사례에도 효과적일 가능성을 시사한다.[161]

3. 2. 도파민 재흡수 억제

고용량의 벤라팍신은 도파민의 재흡수를 약하게 억제한다.[158] 전두엽에는 도파민 수송체가 거의 없기 때문에, 벤라팍신은 이 뇌 영역에서 도파민 신경 전달을 증가시킬 수 있다.[159][160]

3. 3. 기타 수용체에 대한 작용

벤라팍신은 오피오이드 수용체와 α2-아드레날린 수용체에도 간접적으로 영향을 미쳐, 쥐 실험에서 통증 역치를 증가시키는 것으로 나타났다. 이러한 통증 관련 이점은 오피오이드 길항제인 날록손에 의해 역전되어, 오피오이드 메커니즘을 뒷받침한다.[80][14] 벤라팍신은 쥐에서 μ-, κ1-, κ3-, δ- 오피오이드 수용체에 작용하여 통증 역치를 상승시킨다. 벤라팍신과 미르타자핀을 쥐에게 복강 내 투여하고 핫 플레이트 진통계로 통증을 측정한 결과, 두 약물 모두 용량 의존적으로 날록손에 의해 가역적인 항-침해 수용 작용을 보였다. 이는 벤라팍신이 마약성 진통제가 필요한 중증 우울증에도 효과적일 수 있음을 시사한다.[161]

3. 4. 약동학

벤라팍신은 경구 투여 시 흡수가 잘 되며, 경구 투여량의 최소 92%가 전신 순환으로 흡수된다.[3] 간에서 CYP2D6 동종 효소를 통해 데스벤라팍신(''O''-데스메틸벤라팍신)으로 대사되는데, 이는 모 화합물만큼 강력한 SNRI이다.[81] CYP2D6 저대사자는 대사 능력이 떨어져 부작용이 더 심각하게 나타날 수 있다.[82][83]

벤라팍신과 그 대사체의 혈중 농도는 3일 이내에 정상 상태에 도달하며, 치료 효과는 보통 3~4주 이내에 나타난다. 건강한 대상자에게 만성 투여 시 벤라팍신 축적은 관찰되지 않았다.[3]

벤라팍신과 그 대사체의 주요 배설 경로는 신장을 통한다.[3] 벤라팍신의 반감기는 비교적 짧으므로(5시간),[169] 환자는 엄격한 투약 일정을 준수하여 복용량을 놓치지 않도록 해야 한다. 한 번의 복용 누락만으로도 금단 증상이 나타날 수 있다.[84]

벤라팍신은 뇌 밖으로 배출하는 P-글리코프로테인 (P-gp)의 기질이다. P-gp를 암호화하는 유전자인 ABCB1에는 SNP rs2032583이 있으며, 대립 유전자는 C와 T이다. 대다수의 사람 (유럽인의 약 70%와 동아시아인의 90% 정도)은 TT 변형을 가지고 있다.[85] 2007년 연구[86]에 따르면, C 대립 유전자 (CC 또는 CT 변형)를 하나 이상 보유한 보균자는 아미트립틸린, 시탈로프람, 파록세틴 또는 벤라팍신으로 4주 치료 후 관해를 달성할 가능성이 보균자가 아닌 경우보다 7.72배 더 높았다.

4. 처방 용량

일반적으로 처방되는 용량은 하루 75mg에서 225mg 정도이나, 증상이 심각하거나 치료 효과가 없을 경우 그 이상을 사용하기도 한다. 불안 증상이 있는 환자에게는 하루 37.5mg이 처방되기도 한다. 적은 용량을 투여하면 세로토닌 재흡수에만 영향을 주므로, 많은 용량 투여 시 나타나는 노르에피네프린 재흡수 관련 부작용을 막을 수 있다. 반감기가 5시간으로 비교적 짧아, 약물이 서서히 흡수되는 서방형 제제(XR)로 처방되지만, 대한민국에서도 동일 성분의 국내산 제네릭 의약품인 '코팩사 XR 서방캡슐(명인제약)'이 출시된 바 있다.[169] 저용량(150mg/일 미만)에서는 serotoninergic|세로토닌 작용성영어 신경에만 작용하지만, 중등도 용량(150mg/일 이상 300mg/일 이하)에서는 세로토닌 작용계 및 adrenergic|노르아드레날린 작용성영어계에도 작용한다. 고용량(300mg/일 초과)에서는 도파민 작용성 전달에도 영향을 미친다.[112]

5. 부작용

벤라팍신은 다양한 부작용을 유발할 수 있다. 메스꺼움, 과민성 대장 증후군, 현기증, 피로, 불면증, 성기능 장애, 혈압 상승 등은 비교적 흔하게 나타나는 부작용이다.[21]

SNRI 제제인 벤라팍신은 중추 신경계와의 상호작용으로 인해 세로토닌 증후군을 유발할 수 있으며, 심한 경우에는 생명을 위협할 수도 있다.[170] 또한, 벤라팍신은 녹내장 환자의 안압을 증가시킬 수 있으므로, 녹내장 환자는 벤라팍신 복용 시 더 자주 눈 검사를 받아야 할 수 있다.[3]

드물지만 심각한 부작용으로는 간 손상이 나타날 수 있으며, 벤라팍신 중단이 필요할 수 있다. 벤라팍신은 고혈압 환자에게 신중하게 사용해야 하며, 상당한 고혈압이 지속될 경우 벤라팍신 사용을 중단해야 한다.[65][66][67] 2017년 메타 분석에 따르면 벤라팍신의 부작용으로 인한 중단율은 9.4%로 추정된다.[36]

5. 1. 흔한 부작용

메스꺼움, 과민성 대장 증후군, 현기증, 피로, 불면증, 성기능 장애, 혈압 상승 등이 흔한 부작용으로 나타날 수 있다.[21]

5. 2. 세로토닌 증후군

SNRI 제제인 벤라팍신은 중추 신경계와의 상호작용으로 인해 세로토닌 증후군을 유발할 수 있으며, 심한 경우에는 생명을 위협할 수도 있다.[170]

벤라팍신 치료 중, 특히 SSRI 및 SNRI, LSD/LSA, DMT, MDMA, 메스칼린 등의 트립타민, 페네틸아민 계열 환각제, 덱스트로메토르판 (DXM), 트라마돌, 타펜타돌, 페치딘 (메페리딘), 트립탄 등 세로토닌성 약물과 병용하거나, MAOI 등 세로토닌 대사를 손상시키는 약물을 함께 복용하면 잠재적으로 생명을 위협하는 세로토닌 증후군(세로토닌 독성으로 분류되기도 함)[41]이 발생할 수 있다.

세로토닌 증후군 증상으로는 초조, 환각, 혼수 등의 정신 상태 변화, 빈맥, 불안정한 혈압, 고열 등의 자율 신경계 불안정, 과반사, 부조화 등의 신경근 이상, 구역질, 구토, 설사 등의 위장 증상이 나타날 수 있다. 벤라팍신 단독 과다 복용 시에도 벤라팍신 유발 세로토닌 증후군이 보고된 바 있다.[42] 모든 증상이 나타나지는 않지만, 중간 용량(150mg/일)의 벤라팍신 복용 시에도 일부 증상을 동반하는 세로토닌 증후군이 보고되었으며,[43] 저용량(37.5mg/일)에서도 발생한 사례가 있다.[44]

5. 3. 자살 충동 증가

미국 식품의약국(FDA)은 벤라팍신을 포함한 모든 항우울제에 자살 위험 가능성에 대한 블랙 박스 경고를 부착하도록 요구한다. 벤라팍신은 어린이, 청소년 및 젊은 성인에게 금기이다.[26][27][28][29][30][31] 우울증이 있는 어린이 및 청소년의 경우 벤라팍신은 자살 생각 또는 시도의 위험을 증가시킨다.

2014년 성인 우울증 치료를 위한 벤라팍신의 임상 시험 21건에 대한 메타 분석 결과, 위약과 비교했을 때 벤라팍신은 자살 생각 및 행동의 위험을 감소시켰다.[37] FDA 통계학자들의 임상 시험 분석 결과도 벤라팍신을 복용한 성인의 자살 행동 발생률은 플루옥세틴 또는 위약과 유의미한 차이가 없었다.[40]

하지만 핀란드에서 15,000명 이상의 환자를 3.4년 동안 추적한 연구에서는 벤라팍신이 무치료와 비교하여 자살 위험을 60% 증가시켰다(통계적으로 유의미). 반면 플루옥세틴(프로작)은 자살 위험을 절반으로 줄였다.[38]

와이어스(Wyeth)가 후원한 연구에서는 벤라팍신 복용 환자가 플루옥세틴 또는 시탈로프람(셀렉사) 복용 환자보다 자살 위험이 유의하게 더 높았다. 위험 요소를 조정한 후에도 벤라팍신은 플루옥세틴 및 도티에핀과 비교하여 자살 위험 증가와 관련되었지만 통계적으로 유의미하지는 않았다. 자살 시도에 대한 통계적으로 유의미한 더 높은 위험은 조정한 후에도 남아 있었지만, 연구 저자들은 잔류 교란 때문일 수 있다고 결론지었다.[39]

5. 4. 금단 증상

벤라팍신 복용을 중단하는 사람들은 일반적으로 SSRI 금단 증상을 경험한다. 여기에는 불쾌감, 두통, 메스꺼움, 과민성, 감정적 불안정, 전기가 통하는 듯한 느낌 (흔히 "뇌 쇼크"라고 불림[58][59]), 수면 장애 등이 있다.[60] 벤라팍신은 다른 항우울제(SSRI인 파록세틴과 유사)에 비해 중등도에서 심각한 금단 증상의 발생률이 더 높다.[61]

다른 항우울제에 비해 금단 증상의 위험과 심각도가 증가하는 것은 벤라팍신의 짧은 생물학적 반감기와 활성 대사 산물과 관련이 있을 수 있다.[62] 벤라팍신 복용을 중단하면 세로토닌과 노르에피네프린의 수치가 감소하는데, 이러한 금단 증상이 신경 전달 물질 수치가 지나치게 빠르게 감소한 결과일 수 있다는 가설이 제기되었다.[63]

벤라팍신 복용 중단 시 금단 증후군이 발생할 위험은 벤라팍신의 혈중 농도 반감기가 짧기 때문에 높다.[139] 금단 증후군은 파록세틴과 같은 SSRI와 유사하지만 동일하지는 않다. 벤라팍신 복용을 갑자기 중단하면 특히 처음 3일 동안 금단 증상이 나타난다는 연구 결과가 있다.[140] 2001년 ''Drug Safety'' 지에 게재된 정보에 따르면 벤라팍신 중단 시 플루옥세틴으로 전환하여 금단 증상을 억제할 수 있다고 하며, 그 후 단약하는 것이 좋다고 한다.[139] 플루옥세틴(프로작)은 반감기가 길기 때문에 금단 증상을 완화하기 위한 약물 대체에 사용되지만, 한국에서는 출시되지 않았다.

노르에피네프린은 금단 증후군에서 중요한 역할을 한다.[141] 벤라팍신 중단 시 세로토닌과 노르에피네프린이 모두 급속도로 고갈되어 농도가 감소하면서 금단 증상이 유발되는 것으로 보인다.

5. 5. 기타 부작용



벤라팍신은 녹내장 환자의 경우 안압을 증가시킬 수 있으므로, 더 자주 눈 검사를 받아야 할 수 있다.[3]

드물지만 심각한 부작용으로 간 손상이 나타날 수 있으며, 벤라팍신 중단이 필요할 수 있다. 벤라팍신 관련 간 손상의 기전은 불분명하지만, 연구 결과에 따르면 CYP2D6 다형성과 관련이 있을 수 있다.[53]

벤라팍신은 고혈압 환자에게 신중하게 사용해야 하며, 상당한 고혈압이 지속될 경우 벤라팍신 사용을 중단해야 한다.[65][66][67] 또한 바람직하지 않은 심혈관 효과를 나타낼 수 있다.[68]

6. 금기

다음 환자에게는 벤라팍신 투여가 금지되어 있다.[106]


  • 모노아민 산화 효소 억제제(MAO)를 투여 중이거나 투여 중지 후 2주 이내의 환자 (때때로 치명적인 세로토닌 증후군이 발생한다[99][100][125])
  • 중증의 간 기능 장애 (Child-Pugh 분류 C)가 있는 환자
  • 중증의 신 기능 장애 (사구체 여과량 15mL/min 미만)가 있는 환자 또는 투석 중인 환자
  • 제제 성분 (유효 성분, 젤라틴, 셀룰로스, 히프로멜로스, 에틸셀룰로스, 산화철, 이산화 티타늄(IV))에 과민증의 병력이 있는 환자

6. 1. 과민반응

벤라팍신은 과민반응을 보이는 환자에게는 권장되지 않으며, 젤라틴, 셀룰로스, 에틸셀룰로스, 산화철, 이산화 티타늄, 히프로멜로스를 포함한 비활성 성분에 알레르기가 있는 사람도 복용해서는 안 된다.[33][3][34] 잠재적으로 치명적인 세로토닌 증후군을 유발할 수 있으므로 모노아민 산화 효소 억제제(MAOI)와 함께 사용해서는 안 된다.[33][3][34] 벤라팍신은 트라마돌 또는 다른 오피오이드, 트라조돈과 상호작용할 수 있으므로 여러 세로토닌성 제제를 함께 사용할 때는 주의가 필요하다.[35]

다음 환자에게는 투여가 금지되어 있다.[106]

  • 모노아민 산화 효소 억제제(MAO)를 투여 중이거나 투여 중지 후 2주 이내의 환자 (때때로 치명적인 세로토닌 증후군이 발생한다[99][100][125])

6. 2. 모노아민 산화 효소 억제제(MAOI) 복용

모노아민 산화 효소 억제제(MAOI)와 함께 벤라팍신을 복용하면 치명적인 세로토닌 증후군을 유발할 수 있으므로 함께 사용해서는 안 된다.[33][3][34] 모노아민 산화 효소(MAO) 억제제를 투여 중이거나 투여 중지 후 2주 이내의 환자에게는 벤라팍신 투여가 금지된다.[99][100][125]

6. 3. 중증 간/신 기능 장애

중증 간 기능 장애(Child-Pugh 분류 C) 환자나 중증 신 기능 장애(사구체 여과량 15mL/min 미만) 환자, 또는 투석 중인 환자에게는 벤라팍신 투여가 금지된다.[106] 벤라팍신의 드물지만 심각한 부작용 중 하나는 간 손상이다.[53]

6. 4. 소아, 청소년, 젊은 성인

벤라팍신은 어린이, 청소년 및 젊은 성인에게는 투여가 금지되어 있다. 미국 식품의약국(FDA)의 임상 시험 분석 결과,[136] 벤라팍신은 25세 미만 환자에게서 자살 생각 및 행동의 위험을 5배 증가시키는 것으로 나타났다. 다른 분석에서는 7~11세 소아에게는 벤라팍신의 효과가 위약과 동일했으며, 12~17세 청소년에게서는 우울 증상 개선이 나타났다. 그러나 두 그룹 모두 위약에 비해 공격성과 자살 행동이 증가했다.[137] SSRI 치료에 효과를 보지 못한 우울증 청소년을 대상으로 한 연구에서, 벤라팍신 복용군은 다른 SSRI 복용군에 비해 자살률이 상승했다. 특히 자살 시도 경험이 있는 10대의 경우, 벤라팍신으로 전환한 후 자살 시도 및 자해 행위가 다른 SSRI로 전환한 환자보다 유의하게 60% 더 많았다.[138]

6. 5. 임산부

벤라팍신을 임산부에게 사용한 연구는 거의 없으며, 잘 통제된 연구는 드물다. 2010년 5월 캐나다 의학 협회 저널(Canadian Medical Association Journal)에 발표된 연구에 따르면 벤라팍신 사용은 유산 위험을 두 배로 증가시킨다.[45][46] 따라서, 벤라팍신은 임신 중 명확히 필요한 경우에만 사용해야 한다.[3] 2012년에 발표된 국립 선천성 기형 예방 연구(National Birth Defects Prevention Study)의 대규모 환자-대조군 연구에 따르면, 임신 중 벤라팍신 사용과 무뇌증, 구개열, 심실 중격 결손 및 대동맥 축착을 포함한 여러 선천성 기형 사이에 유의미한 연관성이 발견되었다.[47] 전향적 연구에서는 통계적으로 유의미한 선천적 기형은 나타나지 않았다.[48] 그러나 신생아에게서 자가 제한적인 영향이 있다는 보고가 일부 있었다.[49] 다른 세로토닌 재흡수 억제제 (SRIs)와 마찬가지로 이러한 영향은 일반적으로 3~5일 동안 지속되는 단기적이며,[50] 심각한 합병증을 초래하는 경우는 드물다.[51]

6. 6. 양극성 장애

ISBD 실무팀의 보고서에 따르면, 벤라팍신은 양극성 I형 및 II형 환자의 우울증 치료 과정에서 병적으로 고조된 기분 및 행동 상태를 유발할 위험이 특히 높은 것으로 나타났다.[52] 벤라팍신은 SSRI나 부프로피온보다 양극성 장애 환자에게 조증 또는 혼재 에피소드|혼합 에피소드영어를 유발할 가능성이 더 높다.[148] 따라서 의료진은 개별 임상 사례 및 상황을 신중하게 평가하여 벤라팍신 투여 여부를 결정해야 한다.

벤라팍신은 양극성 장애의 주요 우울증 에피소드에 권장되지 않으며 승인되지 않았다. 벤라팍신은 조증 또는 혼합 에피소드를 유발할 위험이 있기 때문이다.[148]

7. 상호작용

세인트 존스 워트를 복용할 때는 벤라팍신 사용에 주의해야 한다.[146] 벤라팍신은 발작 역치를 낮출 수 있으며, 부프로피온, 트라마돌 등과 같이 발작 역치를 낮추는 다른 약물을 함께 복용할 때는 저용량으로 신중하게 사용해야 한다.[147]

7. 1. 세로토닌성 약물

SNRI 제제인 벤라팍신은 세로토닌 증후군을 유발할 수 있으며, 이는 중추 신경계와의 상호작용으로 인해 발생하고, 심하면 생명을 위협할 수 있다.[170]

벤라팍신 치료 중, 특히 SSRI 및 SNRI, 트립타민, 페네틸아민 (LSD/LSA, DMT, MDMA, 메스칼린), 덱스트로메토르판 (DXM), 트라마돌, 타펜타돌, 페티딘 (메페리딘) 및 트립탄을 포함한 세로토닌성 약물과 병용하거나, MAO 억제제를 포함한 세로토닌 대사를 손상시키는 약물과 함께 복용하면 생명을 위협하는 세로토닌 증후군이 발생할 수 있다.[41]

세로토닌 증후군의 증상에는 정신 상태 변화(예: 초조, 환각, 혼수), 자율 신경계 불안정(예: 빈맥, 불안정한 혈압, 고열), 신경근 이상(예: 과반사, 부조화) 또는 위장 증상(예: 구역질, 구토, 설사)이 포함될 수 있다. 벤라팍신 과다 복용 시 벤라팍신 단독으로 복용했을 때 벤라팍신 유발 세로토닌 증후군이 보고된 바 있다.[42] 모든 증상이 나타나지는 않지만, 일부 증상이 나타나는 중단 세로토닌 증후군 상태가 중간 용량(1일 150mg)의 벤라팍신으로 보고되었다.[43] 저용량 벤라팍신(1일 37.5mg)으로 유발된 세로토닌 증후군 환자의 사례도 보고되었다.[44]

7. 2. 발작 역치를 낮추는 약물

부프로피온, 트라마돌과 같이 발작 역치를 낮추는 다른 약물과 벤라팍신을 함께 복용할 때에는 주의해서 저용량으로 투여해야 한다.[90][147]

7. 3. 세인트 존스 워트

세인트 존스 워트를 복용할 때는 벤라팍신 사용에 주의해야 한다.[89] 벤라팍신은 발작 역치를 낮출 수 있으며, 부프로피온, 트라마돌 등과 같이 발작 역치를 낮추는 다른 약물을 함께 복용할 때는 저용량으로 신중하게 사용해야 한다.[90]

벤라팍신은 오락용 약물로 남용될 수 있으며, 한 달 안에 피해가 나타날 수 있다.[91]

벤라팍신은 원래 세계 대부분 지역에서 에펙소(Effexor)라는 브랜드명으로 판매되었다. 일반형 벤라팍신은 2008년경부터, 서방형 벤라팍신은 2010년경부터 시판되었다.[92]

벤라팍신은 전 세계적으로 다양한 브랜드명으로 판매된다.[93] 일부 국가에서는 화이자가 업존(Upjohn)을 분사한 후 비아트리스(Viatris)가 에펙소를 판매한다.[94][95]
