외음절개술 중에는 분만 제2기 동안 아기가 통과할 수 있도록 질구를 확장하기 위해 수술적 절개가 이루어진다.[45][142] 이는 여성에게 시행되는 가장 흔한 의료 절차 중 하나이다. 절개는 피부, 질 상피, 피하 지방, 회음체 및 천회음근을 통해 이루어지며 질에서 항문까지 이어진다.[174][175] 외음절개술은 분만 후 통증을 유발할 수 있으며, 여성들은 종종 절개 수복이나 외음절개술 후 최대 3개월까지 성교 시 통증을 호소한다.[170][171]

질절제술은 질의 전부 또는 일부를 제거하는 수술이며, 일반적으로 악성 종양을 치료하는 데 사용된다.[172] 성기 일부 또는 전부를 제거하면 신경 손상이 발생하고 흉터 또는 유착이 남을 수 있다.[177]

나이가 많고 여러 번 출산 경험이 있는 여성은 질 이완을 수술적으로 교정하기를 선택할 수 있는데, 이 수술은 질 축소 또는 회춘으로 설명되어 왔다.[178] 질 축소 또는 회춘을 받음으로써 여성은 자기 이미지와 성적 쾌락 향상을 경험할 수 있지만,[178] 감염, 질구 협착, 충분하지 않은 축소, 성 기능 저하 (성교 시 통증 포함), 직장 질루를 포함한 위험이 있다.

질 수술은 선택적이거나 미용적일 수 있다. 미용 수술을 받는 여성은 선천적인 질환, 신체적 불편함이 있거나 생식기 외모를 바꾸기를 원할 수 있다. 여러 성전환 수술이 트랜스젠더 사람들에게 제공된다. 모든 간성 상태가 수술 치료를 필요로 하는 것은 아니지만, 일부는 비정형 해부학적 상태를 교정하기 위해 생식기 수술을 선택한다.[181]

7. 다른 동물

질은 암컷이 체내수정되는 동물의 구조이며, 일부 무척추동물에서 사용하는 외상적 수정과는 다르다. 다양한 동물에 대한 질 연구가 특히 부족하지만, 그 위치, 구조 및 크기는 종에 따라 다르다는 것이 기록되어 있다. 태반류 포유류(유태반류와 유대류)에서 질은 자궁에서 암컷의 신체 외부로 이어진다. 암컷 유태반류는 외음부에 두 개의 구멍이 있는데, 하나는 요로를 위한 요도구멍이고 다른 하나는 생식기를 위한 질구멍이다. 종에 따라 이러한 구멍은 내부 요생식동 내부 또는 외부 전정에 있을 수 있다.[243] 암컷 유대류는 두 개의 측면 질을 가지고 있으며, 이는 분리된 자궁으로 이어지지만 둘 다 같은 구멍을 통해 외부로 열린다.[244] 중간 질(median vagina)이라고 알려진 세 번째 관은 일시적이거나 영구적일 수 있으며 출산에 사용된다.[245]
