2. 의학적 용도

경구 투여 시 클로미프라민의 생체이용률은 약 50%이다.[2] 최고 혈장 농도는 경구 투여 후 약 2~6시간(평균 4.7시간)에 나타나며, 56~154ng/mL (178–489nmol/L) 범위이다.[2] 클로미프라민의 정상 상태 농도는 평균 218ng/mL (692nmol/L)이며, 반복 투여 후 7~14일 후에 도달한다.[2] 활성 대사체인 데스메틸클로미프라민의 정상 상태 농도는 약 230–550ng/mL (730–1,750nmol/L)이다.[2] 클로미프라민의 분포 용적 (Vd)은 약 17L/kg이다.[2] 약 97~98%가 혈장 단백질에 결합하며,[2][2] 주로 알부민에 결합한다.[2]

클로미프라민은 주로 간에서 CYP2D6에 의해 대사된다.[2] 종료 반감기는 32시간이며,[152][153] N-데스메틸 대사물질인 데스메틸클로미프라민의 종료 반감기는 약 69시간이다.[2] 클로미프라민은 대부분 배설을 통해 소변 (60%)과 대변 (32%)으로 배출된다.[2]

클로미프라민의 권장 최대 일일 총 복용량은 250mg이지만, 우울증의 치료 저항성 사례[154]와 강박 장애는 255~300mg 범위 내의 용량이 필요할 수 있다. 실제로, 하루 375mg의 용량이 때로는 벤라팍신[155] 또는 아리피프라졸과 병용하여 필요했을 뿐만 아니라 비교적 잘 용인되었다. 그러나 발작은 마프로틸린 외의 다른 모든 삼환계 항우울제보다 클로미프라민에서 발생할 가능성이 높으므로, 일반적으로 권장되는 상한선을 넘어서면 위험이 커지므로 주의가 필요하다. 일일 복용량이 250mg 이하일 경우 발작 발생률은 약 0.48%로 추정할 수 있다.[156] (모든 삼환계 항우울제는 기술적으로 발작 역치를 낮추지만, 이는 아목사핀, 마프로틸린 및 클로미프라민에서만 유의미하다.[157])

클로미프라민의 혈중 농도를 높일 수 있는 심각한 약물 상호 작용이 없는 경우, 25mg에서 150mg 사이의 용량 증가는 50mg 단위로 (공황 장애의 경우 25mg, 조루증[158][159] 및 기면병의 탈력발작)의 경우에는 10~25mg 단위로 조절해야 하며, 150mg 이상에서는 25mg 단위로 조절해야 한다. 우울증(경증 또는 중증), 조루증, 탈력발작-기면증, 강박 장애, 공황 장애 및 발모광에 대한 평균 최적 일일 총 용량은 각각 (밀리그램 단위) 150, 50, 25-75, 150-250, 50-150, 150-200이다. 어떤 사람들은 강박 장애에서 클로미프라민의 최소 최적 치료 용량을 하루 200mg으로 간주하는데, 이는 다른 적응증보다 더 높은 세로토닌 농도를 요구하기 때문이다. 조루증의 경우 클로미프라민은 성관계를 시도하기 3~5시간 전에 필요에 따라 복용할 수 있다.
