2. 의료적 용도

드로스피레논(DRSP)은 단독으로 프로게스토겐 단독 피임약으로 사용되거나, 에티닐에스트라디올(EE) 또는 에스트로테롤(E4)과 같은 에스트로겐과 함께 복합 경구 피임약으로 사용된다.[8] 또한 에스트라디올(E2)과 함께 폐경기 호르몬 요법에도 사용된다.[8]

항미네랄코르티코이드 활성 덕분에, 드로스피레논은 에스트로겐으로 인한 염분 및 수분 저류를 억제하여 체중을 유지하거나 약간 감소시킨다.[28]

2. 1. 피임

드로스피레논은 프로게스토겐 단독 피임약 또는 에티닐에스트라디올, 에스트로테롤 등의 에스트로겐과 함께 복합 경구 피임약으로 사용된다.[8] 저용량 에티닐에스트라디올이 함유된 피임약은 중등도의 여드름, 월경전 증후군(PMS), 월경전 불쾌 장애(PMDD), 월경통 (생리통) 치료에도 사용된다.[38][16]

바이엘은 2009년 야즈를 출시했는데, 28정에 25000KRW 정도이며, 2016년 현재 한국에서 평상시 복용하는 사전피임약 매출액 1위 의약품이다. 2위는 바이엘 머시론이다.

2. 2. 월경 관련 질환 치료

저용량 에티닐에스트라디올 함유 피임약은 여드름, 월경전 증후군(PMS), 월경전 불쾌 장애(PMDD), 월경통(생리통) 치료에 사용된다.[38][16] 에티닐에스트라디올/드로스피레논(EE/DRSP) 복합제는 월경 전 감정적 및 신체적 증상을 줄이고 삶의 질을 향상시키는 것으로 연구되었다.[22][23]

2. 3. 갱년기 호르몬 요법

에스트라디올(E2)과 함께 폐경기 호르몬 요법에 사용되는 경우, E2/DRSP는 중등도에서 중증의 혈관운동 증상 (열감), 질 위축, 폐경기 골다공증을 치료하는 데 특별히 승인되었다.[17][18][19] 이 제형의 드로스피레논 성분은 특히 에스트로겐 유발 자궁내막 증식증을 예방하기 위해 포함된다.[25] E2/DRSP는 골밀도를 증가시키고 골절 발생을 감소시키는 것으로 나타났다.[24][25][26][27] 또한 E2/DRSP는 콜레스테롤 및 트리글리세리드 수치에 긍정적인 영향을 미치고 고혈압이 있는 여성의 혈압을 감소시킨다.[26][27]

3. 금기 사항

드로스피레논은 신부전 또는 만성 신장 질환, 부신 기능 부전, 자궁경부암 또는 기타 호르몬 민감성 암의 존재 또는 병력, 양성 간 종양 또는 악성 간 종양 또는 간 기능 부전, 진단되지 않은 비정상적인 자궁 출혈, 고칼륨혈증(높은 칼륨 수치)이 있는 경우 금기이다.[8][44][43] 신부전, 간 기능 부전 및 부신 기능 부전은 드로스피레논에 대한 노출을 증가시키거나 드로스피레논 사용 시 고칼륨혈증의 위험을 증가시키기 때문에 금기 사항이다.[8]

4. 부작용

드로스피레논 단독 투여 시 1% 이상의 여성에게서 나타날 수 있는 흔한 이상 반응으로는 불규칙한 생리 출혈(돌파 또는 주기 내)(40.3–64.4%), 여드름(3.8%), 자궁 출혈(2.8%), 두통(2.7%), 유방 통증(2.2%), 체중 증가(1.9%), 월경통(1.9%), 구역질(1.8%), 질 출혈(1.7%), 성욕 감소(1.3%), 유방 압통(1.2%), 불규칙한 생리(1.2%) 등이 있다.[8]

야스민과 관련된 사망 사례도 보고되었다. 2012년 2월 춘천의 한 병원에서 월경통을 겪던 환자가 야스민 3개월 처방을 받고 약 한 달 후 가슴 답답함과 호흡 곤란 증상을 보이다 사망했으며, 2016년 5월 인천의 한 산부인과 의원에서 야스민을 처방받아 복용한 여성 환자가 사망했다.[180]

4. 1. 고칼륨혈증

드로스피레논은 칼륨 보존 특성을 가진 항알도스테론제이지만, 대부분의 경우 칼륨 수치의 증가는 예상되지 않는다.[44] 경증 또는 중등도 만성 신장 질환을 앓고 있는 여성이나, 다른 칼륨 보존 약물(ACE 억제제, 안지오텐신 II 수용체 길항제, 칼륨 보존 이뇨제, 헤파린, 항알도스테론제 또는 비스테로이드성 소염 진통제)과 함께 매일 사용하는 경우, 고칼륨혈증 검사를 위해 사용 2주 후에 칼륨 수치를 확인해야 한다.[44][45] 드로스피레논 단독 4mg/일 투여 임상 시험에서, 중단이 필요했던 지속적인 고칼륨혈증은 약 1,000명 중 2명(0.2%)에서 발생했다.[8]

4. 2. 정맥 혈전 색전증 (VTE)

드로스피레논을 포함한 복합 경구 피임약은 심부 정맥 혈전증(DVT)과 폐색전증(PE)을 포함하는 정맥 혈전 색전증(VTE)의 위험을 증가시킨다.[46] 이는 피임약에 함유된 에티닐에스트라디올 성분 때문으로, 응고 인자의 간 단백질 합성에 영향을 주어 혈액 응고 촉진 상태를 유발한다.[49]

피임약을 복용하지 않는 여성의 경우 VTE 발생률은 연간 10,000명당 1~5명 수준이지만, 에티닐에스트라디올 함유 피임약을 복용하면 3~10명으로 증가한다.[50][47] 이는 임신 중(5~20명)이나 산후 기간(40~65명)보다는 낮은 수치이다.[47] 프로게스토겐 단독 피임법, 경피 투여 에스트라디올 (의약품), 폐경 호르몬 요법의 경구 프로게스틴 조합은 VTE 위험 증가와 관련이 없다.[49][48]

에티닐에스트라디올 함유 피임약에 포함된 프로게스틴의 종류에 따라서도 VTE 위험에 차이가 있다.[49] 데소게스트렐, 게스토덴, 드로스피레논, 시프로테론 아세테이트 등을 함유한 피임약은 레보노르게스트렐 함유 피임약보다 VTE 위험이 2~3배 높다는 연구 결과가 있다.[49][50] 그러나 이에 대한 반론도 존재하며, 일부 연구에서는 드로스피레논 함유 피임약과 레보노르게스트렐 함유 피임약 간의 VTE 위험 차이가 발견되지 않았다.[49][50][51][52] 2010년대 중후반의 여러 연구를 종합한 결과, 시프로테론 아세테이트, 데소게스트렐, 드로스피레논, 게스토덴 함유 피임약은 레보노르게스트렐 함유 피임약보다 VTE 위험이 약 1.3~2.0배 높은 것으로 나타났다.[53][54][50]

안드로겐성 프로게스틴은 에티닐에스트라디올의 응고 효과를 어느 정도 상쇄하는 반면,[55][56][57][58] 항안드로겐성 프로게스틴은 그렇지 않다.[49][59]

2010년대 초, 미국 식품의약국(FDA)은 드로스피레논 및 기타 프로게스틴 함유 피임약의 라벨에 혈전 위험 경고를 추가했다.[43]

4. 3. 유방암

드로스피레논은 전임상 연구에서 유방암 세포의 세포 증식 및 세포 이동을 자극하는 것으로 밝혀졌다.[60][69] 그러나 메드록시프로게스테론 아세테이트와 같은 특정 다른 프로게스틴보다 약하게 작용할 수 있다는 증거도 있다.[60][69] 에스트라디올과 드로스피레논의 조합은 폐경기 여성에서 유방 밀도를 증가시키는데, 이는 유방암의 확립된 위험 요인이다.[61][62][63]

드로스피레논과 같은 새로운 프로게스틴을 사용하는 여성의 유방암 위험에 대한 데이터는 현재 부족하다.[64] 프로게스틴 단독 피임법은 일반적으로 유방암 위험이 더 높지 않다.[64] 반면, 에스트로겐과 프로게스틴을 사용한 복합 피임법 및 폐경기 호르몬 요법은 유방암 위험이 더 높은 것과 관련이 있다.[65][64][66]

5. 상호작용

효소 억제제와 효소 유도제는 시토크롬 P450 효소 CYP3A4의 수준과 효능에 영향을 미칠 수 있다.[8] 강력한 CYP3A4 억제제인 케토코나졸을 하루 200mg씩 10일 동안 투여하면 드로스피레논에 대한 노출이 2.0~2.7배 증가하는 것으로 나타났다.[8][123] 드로스피레논은 오메프라졸(CYP2C19를 통해 대사됨), 심바스타틴(CYP3A4를 통해 대사됨), 미다졸람(CYP3A4를 통해 대사됨)의 대사에 영향을 미치지 않으며, 이러한 대사 경로를 통해 대사되는 다른 약물의 대사에도 영향을 미치지 않을 가능성이 높다.[8][123]

드로스피레논은 칼륨 보존 약물(예: ACE 억제제, 안지오텐신 II 수용체 길항제, 칼륨 보존 이뇨제, 칼륨 보충제, 헤파린, 항광질코르티코이드, 비스테로이드성 소염 진통제)과 상호 작용하여 칼륨 수치를 더욱 증가시킬 수 있다.[8][123] 이는 고칼륨혈증(높은 칼륨 수치)의 위험을 증가시킬 수 있다.[8][123]

드로스피레논은 안드로겐과 같은 테스토스테론 및 디히드로테스토스테론(DHT)의 생물학적 표적인 AR의 길항제이다.[7][2] 합성 아나볼릭 스테로이드 메트리볼론에 대한 AR 친화력의 약 1~65%를 가지고 있다.[7][2][3][69] 스피로노락톤보다 더 강력한 항안드로겐이지만, 사이프로테론 아세테이트보다 덜 강력하며, 동물에서 약 30%의 항안드로겐 활성을 나타낸다.[7][84][79][73]

드로스피레논은 여성에게 3mg/일 투여 시, HDL 콜레스테롤과 트리글리세리드 수치를 높이고, LDL 콜레스테롤 수치를 낮추는 등 혈청 지질 프로필에 항안드로겐 효과를 나타낸다.[2] 이 약물은 성호르몬 결합 글로불린(SHBG)과 혈청 지질에 대한 에티닐에스트라디올의 효과를 억제하지 않으며, 레보노르게스트렐과 같은 안드로겐성 프로게스틴과는 달리, 사이프로테론 아세테이트와 같은 다른 항안드로겐성 프로게스틴과 유사하다.[2][7][86] SHBG 수치는 드로스피레논에 비해 사이프로테론 아세테이트의 더 강력한 항안드로겐 활성으로 인해 에티닐에스트라디올과 사이프로테론 아세테이트가 에티닐에스트라디올과 드로스피레논보다 유의하게 더 높다.[87]

6. 2. 약동학

드로스피레논의 경구 투여 생체이용률은 66%에서 85% 사이이다.[7][2][3] 경구 투여 후 1~6시간 사이에 최대 혈중 농도가 나타난다.[7][2][8][89] 단일 4mg 투여 후 혈중 농도는 약 27ng/mL이다.[8] 지속적으로 투여하면 드로스피레논 수치는 1.5~2배 축적되며, 투여 7~10일 후에 정상 상태에 도달한다.[7][8][2] 1일 4mg 드로스피레논 투여 시 정상 상태에서 최대 혈중 농도는 약 41ng/mL이다.[8] 에티닐에스트라디올 30μg/일과 드로스피레논 3mg/일을 함께 투여하면 단일 투여 후 드로스피레논의 최대 혈중 농도는 35ng/mL이며, 정상 상태에서는 최대 60~87ng/mL, 최저 혈중 농도는 20~25ng/mL이다.[2][7] 1~10mg의 단일 용량에서 경구 드로스피레논의 약동학은 선형적이다.[8][2] 음식과 함께 섭취해도 드로스피레논의 흡수에는 영향을 미치지 않는다.[8]

드로스피레논의 분포 반감기는 약 1.6~2시간이다.[2][7] 겉보기 분포 용적은 약 4L/kg이다.[8] 혈장 단백질 결합은 95~97%이며, 알부민에 결합한다.[8][7] 3~5%는 자유롭게 또는 결합되지 않은 상태로 순환한다.[8][7] 성 호르몬 결합 글로불린(SHBG)이나 코르티코스테로이드 결합 글로불린(CBG)에 대한 친화력은 없으므로 순환계에서 이러한 혈장 단백질에 결합하지 않는다.[7]

드로스피레논은 광범위하게 대사된다.[2] 락톤 고리가 열리면서 산 형태로 대사되며,[7][8] C4와 C5 위치 사이의 이중 결합의 환원과 그에 따른 황산화를 통해서도 대사된다.[7][8] 주요 대사 산물은 드로스피레논 산과 4,5-디히드로드로스피레논 3-황산염이며, 둘 다 시토크롬 P450 시스템과 독립적으로 형성된다.[8][2] 이 두 대사 산물은 모두 생물학적 활성이 없다.[8] 드로스피레논은 CYP3A4에 의한 산화 대사도 겪는다.[8][2][5][6]

배설을 통해 소변과 대변으로 배출되며, 소변보다 대변으로 더 많이 배설된다.[8] 변화되지 않은 드로스피레논은 소변과 대변에서 미량만 발견된다.[8] 최소 20가지의 서로 다른 대사체가 소변과 대변에서 확인된다.[2] 드로스피레논과 그 대사체는 소변에서 약 38%가 글루쿠론산 접합체, 47%가 황산염 접합체, 10% 미만이 비접합 형태로 배설된다.[2] 대변에서는 약 17%가 글루쿠론산 접합체, 20%가 황산염 접합체, 33%가 비접합 형태로 배설된다.[2]

제거 반감기는 25~33시간이다.[8][2][7] 드로스피레논의 반감기는 반복 투여해도 변하지 않는다.[8] 드로스피레논의 제거는 마지막 투여 후 10일 이내에 거의 완료된다.[2][8]

7. 화학적 특성

드로스피레논(1,2-디하이드로스피로레논 또는 17β-하이드록시-6β,7β:15β,16β-디메틸렌-3-옥소-17α-프레그-4-엔-21-카르복실산, γ-락톤)은 합성 스테로이드성 17α-스피로락톤 유도체로, 간단히 스피로락톤이라고도 한다.[103][104] 스피로노락톤, 칸레논, 스피로레논과 같은 다른 스피로락톤과 유사체이다.[103][104]

드로스피레논은 스피로노락톤에서 C7α 아세틸 티오 치환이 제거되고, C6β–7β 및 C15β–16β 위치에 두 개의 메틸렌 그룹이 치환된 구조를 가진다.[90]

드로스피레논 및 스피로락톤과 같은 스피로락톤은 프로게스토겐 및 항광물 코르티코이드 활성을 갖는 프로게스테론의 유도체이다.[91][92][93] 스피로노락톤(프로게스토겐 활성이 미미한 화합물)의 C7α 아세틸티오 그룹이 제거되면서,[94][95] 드로스피레논에서는 프로게스토겐 활성이 회복된 것으로 보인다. C7α 치환이 없는 스피로노락톤 유사체인 SC-5233은 드로스피레논과 유사하게 강력한 프로게스토겐 활성을 갖는다.[96]

8. 역사

드로스피레논은 1976년에 특허를 받았으며 2000년에 의료용으로 도입되었다.[99][12] 독일의 쉐링 AG(Schering AG)는 드로스피레논 생산에 대한 여러 특허를 받았는데, 여기에는 1998년과 2000년에 각각 허가된 WIPO 및 미국 특허가 포함된다.[97][98] 2000년, 드로스피레논은 에티닐에스트라디올과 함께 복합 경구 피임약으로 의료용으로 도입되었다.[99] 드로스피레논은 도입 시기를 기준으로 "4세대" 프로게스틴으로 묘사되기도 한다.[100][101] 2005년에는 폐경기 호르몬 요법에 에스트라디올과 함께 사용하는 것이 승인되었다.[17] 2019년에는 프로게스토겐 단독 피임약으로 사용하도록 도입되었다.[8] 2021년에는 에스테트롤과 드로스피레논을 함유한 복합 경구 피임약이 승인되었다.[102]

9. 사회와 문화

드로스피레논은 에티닐에스트라디올과 함께 야스민 및 야즈, 에스테트롤과 함께 Nextstellis, 에스트라디올과 함께 Angeliq 등 다양한 브랜드명으로 전 세계에서 판매된다.[103][102]

바이엘은 2009년 야스민의 후속으로 야즈를 한국에 출시했다. 야즈는 28정 기준 약 25,000원에 판매되었으며, 2016년 기준 한국에서 평상시 복용하는 사전피임약 매출액 1위를 기록했다.[1] 2위는 바이엘의 머시론이다.[1]

2012년 2월 춘천의 한 병원에서 월경통 환자가 야스민을 3개월 처방받은 후 약 한 달 만에 가슴 답답함과 호흡 곤란 증세를 보이다 사망했다. 2016년 5월에는 인천 검단지역의 한 산부인과에서 야스민을 처방받은 여성이 사망하는 사건이 발생했다.[180]

바이엘은 드로스피레논과 특정 프로게스틴을 함유한 복합 경구 피임약에서 정맥 혈전 색전증(VTE) 위험이 레보노르게스트렐 함유 복합 경구 피임약보다 높다는 이유로 많은 소송에 직면했다.[51]

2012년 7월, 바이엘은 야즈, 야스민 및 기타 드로스피레논 함유 피임약과 관련된 12,000건 이상의 소송이 진행 중이라고 주주들에게 알렸다. 당시까지 1,977건의 사건에 대해 총 4.26억달러(사건당 평균 212000USD)를 지불했으며, 나머지 사건 해결을 위해 6.105억달러를 추가로 배정했다.[109]

2015년 7월 17일 기준, 드로스피레논 관련 VTE 소송 및 청구는 최소 4,000건 이상 계류 중이었다. 이는 바이엘이 이미 책임을 인정하지 않고 합의한 약 10,000건의 청구(19.7억달러)에 더해진 수치다. 바이엘은 뇌졸중 및 심장마비를 포함한 동맥 혈전 색전증 사건에 대해서도 5690만달러에 합의했다.[110]

드로스피레논은 이 약물의 일반명이며, 국제 일반명(INN), 미국 채택 명칭(USAN), 영국 승인 명칭(BAN), 일본 승인 명칭(JAN)으로 사용된다. 프랑스어 명칭(DCF)은 ''drospirénone''이다.[103] 드로스피레논은 ''1,2-dihydrospirorenone'' 또는 ''dihydrospirenone''의 이름을 줄인 형태이다.[103][104]

드로스피레논은 전 세계적으로 다양한 브랜드명으로 에스트로겐과 함께 판매된다.[103] 특히 에티닐에스트라디올과 함께 야스민 및 야즈, 에스테트롤과 함께 Nextstellis, 에스트라디올과 함께 Angeliq라는 브랜드명으로 판매된다.[103][102]

10. 연구

에티닐에스트라디올, 드로스피레논 및 프라스테론(ethinylestradiol/drospirenone/prasterone) 조합은 Pantarhei Bioscience에서 여성의 임신 예방을 위한 복합 경구 피임약으로 개발 중이다.[111] 이 조합에는 경구용 안드로겐 전구체인 프라스테론(데히드로에피안드로스테론, DHEA)이 포함되어 테스토스테론(testosterone)을 대체하고 에티닐에스트라디올과 드로스피레논에 의한 테스토스테론 억제로 인한 테스토스테론 결핍을 피한다.[111] 2018년 8월 현재, 이 제형은 2상/3상 임상 시험 중에 있다.[111]

드로스피레논은 남성 호르몬 피임법에서 프로게스틴으로 잠재적으로 사용될 수 있다고 제안되었다.[112]
