1. 개요

2. 역사

최초로 합성된 클로르디아제폭시드의 분자 구조


최초의 벤조디아제핀인 클로르디아제폭시드(리브리엄)는 1955년 호프만 라 로슈의 레오 스턴바흐가 진정제를 개발하다가 합성했다.[342][343] 1957년, 실험실 정리 중 이 화합물이 발견되었고, 동물 실험 결과 매우 강한 진정, 항경련, 근육 이완 효과를 보였다. 1960년 '리브리엄'이라는 상품명으로 전 세계에 출시되었다.[342][343]

1960년부터 호프만-라 로슈에서 판매되었으며 리브리엄이라는 상표로 판매된 최초의 벤조디아제핀인 클로르디아제폭사이드의 분자 구조.


호프만 라 로슈는 1963년 디아제팜(바리움)을 출시하여 상업적 성공을 이어갔다.[344] 1970년대 벤조디아제핀은 바르비투르산염을 대체하며 진정 및 최면제로 널리 사용되었다.[344]

1980년대 벤조디아제핀의 중독 위험성이 밝혀지면서, 영국에서 1만 4천 명의 환자들이 제약사를 상대로 "중독성을 알면서도 의사에게 정보를 알리지 않았다"는 이유로 집단 소송을 제기했다.[345]

항우울제, 비벤조디아제핀계 약물의 등장에도 불구하고, 벤조디아제핀은 여전히 단기 불안 완화 목적으로 사용되고 있다.[346]

3. 의료 용도

벤조디아제핀은 진정, 수면, 항불안, 항경련, 근육 이완 및 기억상실 작용을 나타낸다.[349][350] 이러한 특성으로 알코올 의존증, 발작, 불안 장애, 공황 장애, 흥분, 불면증 등 다양한 질환에 사용된다.[351] 대부분 경구 투여되지만, 정맥 주사, 근육 주사, 직장 투여도 가능하다.[11]

일반적으로 벤조디아제핀은 단기간 사용 시 안전하고 효과적이지만,[12][13] 장기간 사용 시에는 내성, 의존성, 금단 증상, 인지 기능 저하 등의 문제가 발생할 수 있다.[14][15] 장기 사용 또는 오용은 인지 결핍, 우울증, 불안을 유발하거나 악화시킬 수 있다.[16][221]

3. 1. 공황 장애

벤조디아제핀은 효과, 내약성 및 빠른 항불안제 작용 발현으로 인해 공황 장애와 관련된 불안 치료에 자주 사용된다.[18] 그러나, 공황 장애에 대한 벤조디아제핀의 장기적인 사용에 관해서는 전문가 단체 간에 의견 불일치가 있다. 벤조디아제핀이 장기적으로 효과가 없으며[21] 치료 저항성 사례에만 사용해야 한다고 주장하는 견해[19]부터 선택적 세로토닌 재흡수 억제제(SSRI)만큼 장기적으로 효과가 있다고 주장하는 견해[20]까지 다양하다.

미국 정신 의학회(APA) 지침은 2009년 1월에 발표되었으며,[20] 일반적으로 벤조디아제핀은 내약성이 좋으며 공황 장애의 초기 치료에 사용되는 것은 수많은 대조 시험에 의해 강력하게 뒷받침된다고 언급했다. APA는 기존의 공황 장애 치료법 중에서 다른 치료법보다 더 권장할 만한 충분한 증거가 없다고 밝히고 있다. 벤조디아제핀, SSRI, 세로토닌-노르에피네프린 재흡수 억제제(SNRI), 삼환계 항우울제, 심리 치료 중 치료법 선택은 환자의 병력, 선호도 및 기타 개별 특성을 기반으로 해야 한다. 선택적 세로토닌 재흡수 억제제는 공황 장애 환자에게 최선의 약물 요법이 될 가능성이 높지만 벤조디아제핀도 자주 사용되며 일부 연구에 따르면 이러한 약물이 SSRI보다 더 자주 사용된다고 한다. 벤조디아제핀의 한 가지 장점은 항우울제보다 불안 증상을 훨씬 더 빠르게 완화한다는 것이며, 따라서 빠른 증상 조절이 중요한 환자에게 선호될 수 있다. 그러나 이러한 장점은 벤조디아제핀 의존성이 발생할 가능성으로 상쇄된다. APA는 우울 증상 또는 최근 물질 사용 장애 병력이 있는 사람에게 벤조디아제핀을 권장하지 않는다. APA 지침은 일반적으로 공황 장애에 대한 약물 요법을 최소 1년 동안 지속해야 하며, 임상 경험에 따르면 재발을 방지하기 위해 벤조디아제핀 치료를 계속하는 것이 뒷받침된다고 명시하고 있다. 벤조디아제핀 내성 및 금단에 대한 심각한 우려가 제기되었지만, 벤조디아제핀을 장기간 사용하는 환자에서 용량 증가가 유의미하게 나타났다는 증거는 없다. 이러한 많은 환자에게서 벤조디아제핀의 안정적인 용량이 수년 동안 효능을 유지한다.[20]

영국 국립 보건 임상 연구소(NICE)에서 발표한 지침은 다른 방법론을 사용하여 체계적인 검토를 수행했으며 다른 결론에 도달했다. 이들은 위약 대조군 연구가 아닌 연구의 정확성에 의문을 제기했다. 그리고 위약 대조군 연구의 결과에 근거하여 내성과 신체적 의존성이 빠르게 발생하고 6주 이상 사용 후 반동 불안을 포함한 금단 증상이 나타나기 때문에 2~4주 이상 벤조디아제핀 사용을 권장하지 않는다.[21][22] 그럼에도 불구하고 벤조디아제핀은 여전히 불안 장애의 장기 치료에 처방되고 있으며, 특정 항우울제와 심리 치료가 1차 치료 옵션으로 권장되며, 항경련제 프레가발린은 2차 또는 3차 치료로 표시되고 장기 사용에 적합하다.[23] NICE는 광장 공포증을 동반하거나 동반하지 않는 공황 장애에 대한 벤조디아제핀의 장기 사용은 허가되지 않은 적응증이며, 장기적인 효능이 없으므로 임상 지침에서 권장하지 않는다고 밝혔다. 인지 행동 치료와 같은 심리 치료는 공황 장애의 1차 치료법으로 권장되며, 벤조디아제핀 사용은 이러한 치료법의 치료적 효과를 방해하는 것으로 밝혀졌다.[21]

벤조디아제핀은 일반적으로 경구 투여되지만, 매우 드물게 공황 발작 치료를 위해 로라제팜 또는 디아제팜을 정맥 투여할 수 있다.[11]

3. 2. 범불안 장애

벤조디아제핀은 범불안 장애(GAD)의 단기 관리에 효과적이지만, 장기적인 개선 효과는 미미하다.[24] 영국 국립보건임상연구원(NICE)은 벤조디아제핀을 범불안 장애의 즉각적인 관리에 사용할 수 있지만, 2~4주 이상 투여는 권장하지 않는다.[25] NICE가 범불안 장애의 장기적인 관리를 위해 권장하는 유일한 약물은 항우울제이다.[258]

캐나다 정신의학 협회(CPA)는 다른 두 가지 항우울제 치료가 실패한 경우에만 알프라졸람, 브로마제팜, 로라제팜, 디아제팜 등의 벤조디아제핀을 2차 선택으로 권장한다. 벤조디아제핀은 심각한 불안과 초조함을 완화하기 위해 제한된 시간 동안 사용할 수 있다. CPA 가이드라인에 따르면 4~6주 후에는 벤조디아제핀의 효과가 위약 수준으로 감소할 수 있으며, 벤조디아제핀이 범불안 장애의 핵심 증상인 반추적 걱정을 완화하는 데 항우울제보다 덜 효과적이라고 한다. 그러나 경우에 따라 항우울제에 추가하여 벤조디아제핀으로 장기간 치료하는 것이 정당화될 수 있다.[26][259]

3. 3. 불면증

벤조디아제핀은 불면증의 단기 치료에 유용하지만, 의존성 위험 때문에 2~4주 이상 사용은 권장되지 않는다. 벤조디아제핀은 수면 시간을 늘리고 각성을 줄이지만, 수면의 질을 악화시킬 수 있다. 비벤조디아제핀계 수면제(Z-drug)가 벤조디아제핀보다 나은지는 불분명하다. 고령자는 다른 치료법이 실패하지 않는 한 벤조디아제핀 사용을 피해야 한다.

3. 4. 발작

지속적인 경련성 간질 발작은 응급 의료가 필요한 상태이며, 일반적으로 강력한 항간질약인 속효성 벤조디아제핀 투여로 효과적으로 대처할 수 있다. 병원 환경에서는 클로나제팜, 로라제팜 또는 디아제팜 정맥 투여가 1차 선택 약물이 된다. 클로나제팜은 다른 약물보다 강력한 항경련 작용이 있고, 로라제팜은 작용 개시가 빠르며, 디아제팜은 작용 시간이 길다[265].[266] (일본에서는 디아제팜에만 주사제가 존재한다). 일반적인 환경에서는 정맥 내 투여가 실용적이지 않으므로, 좌제에 의한 직장으로부터의 디아제팜 투여나 미다졸람의 비강 또는 구강 점막 흡수가 사용되며, 미다졸람이 투여가 더 쉽고 사회적으로 더 허용된다.

3. 5. 알코올 금단

클로르디아제폭사이드는 알코올 해독에 가장 일반적으로 사용되는 벤조디아제핀[41]이지만, 디아제팜을 대체 약물로 사용할 수도 있다. 두 약물 모두 금주하려는 의지가 있는 사람들의 해독에 사용되며, 벤조디아제핀 자체에 대한 내성과 의존성 발달 위험을 줄이기 위해 단기간 처방된다.[11] 반감기가 긴 벤조디아제핀은 해독 과정을 더 견딜 만하게 만들고, 위험한(그리고 잠재적으로 치명적인) 알코올 금단 증상이 발생할 가능성을 줄여준다. 반면에, 단시간 작용 벤조디아제핀은 돌파 발작을 일으킬 수 있으므로 외래 환자 환경에서의 해독에는 권장되지 않는다. 옥사제팜과 로라제팜은 약물 축적 위험이 있는 환자, 특히 노인 및 간경변 환자에게 자주 사용되는데, 이들은 글루쿠론산 결합을 통해 다른 벤조디아제핀과는 다른 방식으로 대사되기 때문이다.[42][43]

벤조디아제핀은 알코올 금단 증후군 관리, 특히 발작의 위험한 합병증 예방 및 치료, 심각한 섬망 진정에 선호되는 선택지이다.[44] 로라제팜은 예측 가능한 근육 주사 흡수를 보이는 유일한 벤조디아제핀이며, 급성 발작을 예방하고 조절하는 데 가장 효과적이다.[45]

3. 6. 기타 적응증

벤조디아제핀은 다양한 질환에 처방된다.

  • 기계 환기를 받는 환자나 극심한 고통을 겪는 환자를 진정시킬 수 있다. 이 경우 호흡 억제의 위험이 있으므로 주의해야 하며, 벤조디아제핀 과다 복용 치료 시설을 이용할 수 있어야 한다.[46] 또한 벤틸레이터에서 제거된 후 나중에 외상 후 스트레스 장애(PTSD)가 발생할 가능성을 높이는 것으로 밝혀졌다.[47]
  • 특히 다른 치료법으로 증상이 적절하게 조절되지 않는 진행성 질환에서 호흡 곤란(숨 가쁨) 관리에 사용될 수 있다.[48]
  • 수술 전 몇 시간 전에 불안을 완화하기 위해 투여하는 약물로 효과적이다. 또한 기억 상실을 유발하여, 환자가 시술의 불쾌한 기억을 떠올리지 못하게 하는 데 유용할 수 있다.[49] 치과 공포증이 있는 환자와 굴절 수술과 같은 일부 안과 시술에도 사용되지만, 매우 불안한 환자에게만 권장된다.[50] 미다졸람은 강력한 진정 작용과 빠른 회복 시간, 주사 시 통증을 줄여주는 수용성으로 인해 이 용도로 가장 일반적으로 처방된다. 디아제팜과 로라제팜이 사용되기도 한다. 로라제팜은 특히 기억 상실이 원하는 효과일 때 더 효과적일 수 있는 뚜렷한 기억 상실 특성을 가지고 있다.[11]
  • 강력한 근육 이완 작용으로 잘 알려져 있으며 근육 경련 치료에 유용할 수 있다.[11] 그러나 내성이 종종 근육 이완 효과에 대해 발생한다.[16] 바클로펜[51] 또는 티자니딘이 벤조디아제핀의 대안으로 사용되기도 한다. 티자니딘은 디아제팜과 바클로펜에 비해 내약성이 우수한 것으로 밝혀졌다.[52]
  • 환각제 중독으로 인한 급성 공황을 치료하는 데 사용된다.[53] 또한 급성 흥분 상태의 개인을 진정시키는 데 사용되며 필요한 경우 근육 주사로 투여할 수 있다.[54] 때로는 급성 정신병과 같은 정신과적 응급 상황, 조현병 또는 조증에 대한 단기 치료에 효과적일 수 있으며, 리튬 또는 신경이완제(항정신병제)의 효과가 나타날 때까지 신속한 진정 및 진정을 가져온다. 로라제팜이 가장 일반적으로 사용되지만, 클로나제팜은 급성 정신병 또는 조증에 대해 처방되기도 한다.[55] 의존성 위험으로 인해 장기간 사용은 권장되지 않는다.[11] 급성 정신병 치료를 위한 벤조디아제핀 단독 사용 및 항정신병 약물과의 병용 사용에 대한 추가 연구가 필요하다.[56]
  • 클로나제팜은 많은 형태의 수면 발작을 치료하는 데 사용된다.[57] 렘 수면 행동 장애는 저용량의 클로나제팜에 잘 반응한다.[58][59] 하지 불안 증후군은 클로나제팜 사용이 아직 연구 중이므로 세 번째 치료 옵션으로 클로나제팜을 사용하여 치료할 수 있다.[60][61]
  • 때때로 강박 장애(OCD)에 사용되지만, 일반적으로 이 적응증에는 효과가 없는 것으로 간주된다. 그러나 한 소규모 연구에서 효과가 발견되었다.[62] 벤조디아제핀은 치료 저항성 사례에서 치료 옵션으로 고려될 수 있다.[63]
  • 항정신병제는 일반적으로 섬망에 대한 1차 치료법이다. 그러나 섬망이 알코올 또는 진정 수면제 약물 금단으로 인해 발생하는 경우 벤조디아제핀이 1차 치료법이다.[64]
  • 저용량의 벤조디아제핀이 전기 충격 요법의 부작용을 감소시킨다는 몇 가지 증거가 있다.[65]

4. 부작용

벤조디아제핀의 일반적인 부작용은 진정 및 근육 이완 작용과 관련이 있으며, 졸음, 현기증, 민첩함 및 집중력 감소를 포함한다.[354][355] 운동 협응 능력 부족은 특히 노인에게서 낙상 및 부상을 유발할 수 있다.[88][89][90] 운전 능력 저하 및 교통사고 가능성 증가도 나타날 수 있다.[91][92] 성욕 감소 및 발기 부전은 흔한 부작용이며, 우울증과 탈억제가 나타날 수 있다. 저혈압 및 호흡 억제(과소 환기)는 정맥 내 투여 시 발생할 수 있다.[88][89] 덜 흔한 부작용으로는 메스꺼움, 식욕 변화, 흐린 시야, 혼란, 행복감, 이인증, 악몽 등이 있다. 간 독성 사례는 매우 드물게 보고되었다.[11][93]

장기 복용 시에는 인지 장애, 정동 및 행동 문제가 발생할 수 있다. 혼란스러운 감정, 사고력 저하, 성욕 상실, 광장 공포증, 사회 공포증, 불안 및 우울증 증가, 여가 활동 및 관심사 상실, 감정 표현의 어려움 등이 나타날 수 있지만, 모든 장기 복용자에게서 이러한 문제가 발생하는 것은 아니다.[94][95] 자신, 환경 및 관계에 대한 인식 변화도 발생할 수 있다.[99]

그 외에도 다음과 같은 부작용들이 보고되었다.


  • 뇌 수축: 1982년 영국 의학연구심의회(MRC)는 벤조디아제핀 복용이 만성 알코올 중독자에게서 나타나는 뇌 수축 현상과 유사한 부작용을 유발할 수 있다는 연구 결과를 발표했으나, 이후 별다른 조치를 취하지 않았다.[358]
  • 알츠하이머병: 영국 의학저널(BMJ)에 게재된 보고서에 따르면 벤조디아제핀은 알츠하이머병 발병률을 평균 43~51% 증가시킨다.[359]
  • 자살 충동: 벤조디아제핀은 우울증 환자의 자살 충동을 유발할 수 있다.[360]
  • 알코올 및 중추신경억제제와의 병용 투여: 알코올/중추신경억제제와 병용 투여 시 중증 진정 작용, 호흡기계 및 심혈관계 억제를 포함한 임상 효과가 증강될 수 있으며, 혼수 또는 사망 위험을 높일 수 있다.
  • 마약류와의 병용 투여: 마약류와 병용 투여 시 깊은 진정, 호흡 억제, 혼수상태 및 사망을 초래할 수 있다.

4. 1. 인지 기능에 대한 영향

벤조디아제핀의 단기 사용은 인지 기능의 여러 영역에 부정적인 영향을 미치며, 가장 두드러진 영향은 새로운 정보의 기억 형성과 강화를 방해하여 완전한 전진성 기억 상실을 유발할 수 있다는 것이다.[88] 그러나 연구자들은 장기 투여의 영향에 대해 상반된 의견을 가지고 있다. 한 가지 견해는 단기적인 영향의 많은 부분이 장기적으로 지속되고 심지어 악화될 수 있으며, 벤조디아제핀 사용을 중단한 후에도 해결되지 않는다는 것이다. 다른 견해는 만성 벤조디아제핀 사용자의 인지 기능 저하는 투여 후 짧은 기간 동안만 발생하거나, 이러한 결손의 원인이 불안 장애라는 것이다.

확실한 연구가 부족하지만, 전자의 견해는 2004년 13개의 소규모 연구에 대한 메타 분석에서 지지를 받았다.[99][100] 이 메타 분석은 벤조디아제핀의 장기 사용이 인지 기능의 모든 영역에 중등도에서 큰 수준의 부작용을 나타내며, 특히 시공간적 기억력에 가장 흔하게 손상이 나타난다는 것을 발견했다. 보고된 다른 손상으로는 IQ 감소, 시운동 협응력 저하, 정보 처리 능력 저하, 언어 학습 능력 저하 및 집중력 저하 등이 있었다. 메타 분석의 저자[100]와 후기 검토자[99]는 이 메타 분석의 적용 가능성이 제한적이라고 지적했는데, 이는 대상자들이 주로 금단 클리닉에서 추출되었고, 공존하는 약물, 알코올 사용, 정신 질환이 명확하게 정의되지 않았으며, 포함된 연구 중 일부가 금단 기간 동안 인지 평가를 수행했기 때문이다.

4. 2. 역설적 반응

벤조디아제핀은 드물게 공격성, 폭력, 충동성, 과민성 및 자살 행동과 같은 역설적 반응을 유발할 수 있다.[5] 이러한 반응은 탈억제와 그에 따른 사회적으로 용납할 수 없는 행동에 대한 통제력 상실의 결과로 설명된다.[5] 역설적 반응은 일반 인구에서는 드물게 발생하며, 발병률이 1% 미만으로 위약과 유사하다.[102][103]

그러나 향정신성 약물을 오용하는 사람, 경계성 성격 장애가 있는 사람, 어린이 및 고용량 요법을 받는 환자에게서 더 자주 발생한다.[104][105] 이러한 집단에서 충동 조절 문제는 탈억제의 가장 중요한 위험 요소일 것이다.[103] 학습 장애 및 신경 질환 또한 중요한 위험 요소이다.[103] 탈억제에 대한 대부분의 보고는 고강도 벤조디아제핀의 고용량과 관련이 있다.[103] 역설적 효과는 벤조디아제핀의 만성 사용 후에도 나타날 수 있다.[106]

5. 오남용

벤조디아제핀은 주요 중독성 물질로 간주된다.[212] 국제적으로 벤조디아제핀은 스케줄 IV 통제 약물로 분류되지만, 플루니트라제팜은 예외적으로 스케줄 III 약물이다.[214] 대한민국에서는 마약류 관리법 제2조 제4호 다, 라목에 따라 오남용 우려 의약품으로 분류된다.

벤조디아제핀은 기분 전환 목적으로 오남용될 수 있다.[218] 벤조디아제핀과 알코올을 함께 복용하면 블랙아웃, 위험 감수 행동, 발작 및 과다 복용의 위험이 증가한다.[220] 특히, 알코올과 함께 복용 시 사망 위험이 더 높아진다.[219]

벤조디아제핀에 대한 의존성과 내성은 약물을 오용하는 사람들 사이에서 빠르게 나타날 수 있으며, 불과 3주 연속 사용 후에도 금단 증후군이 나타날 수 있다.[221] 장기간 사용은 신체적, 정신적 의존성과 우울증, 불안, 공황 발작, 광장 공포증과 같은 심각한 금단 증상을 유발할 수 있다.[221]

일화적인 증거에 따르면, 테마제팜은 가장 심리적으로 습관성(중독성)이 강한 벤조디아제핀일 수 있다.[226] 비의학적 테마제팜 사용은 세계 일부 지역, 특히 유럽과 호주에서 유행병 수준에 이르렀으며, 많은 동남아시아 국가에서 주요 중독성 물질이다.[225] 이로 인해 여러 국가에서는 테마제팜을 더 제한적인 법적 지위에 두거나, 스웨덴처럼 완전히 금지하기도 했다.[225]

6. GABA 수용체

벤조디아제핀은 뉴런의 흥분성을 감소시키는 GABA의 효과를 증가시켜 작용한다.[167] GABA는 GABAA 수용체에 결합하여 뉴런의 흥분성을 조절한다.[167]

GABAA 수용체는 뉴런 간의 화학적 시냅스에 위치한 다중 단백질 복합체이다. 모든 GABAA 수용체는 뉴런 세포막을 가로질러 염소 이온을 전달하는 이온 채널과 신경전달물질 GABA에 대한 두 개의 결합 부위를 포함하며, 일부는 벤조디아제핀에 대한 단일 결합 부위를 포함한다. 벤조디아제핀은 GABAA 수용체의 α와 γ 소단위체의 인터페이스에서 결합한다. 벤조디아제핀은 GABAA 수용체를 GABA에 대한 친화력이 더 큰 형태로 고정시켜 염소 이온 채널의 개방 빈도를 증가시킨다. 이렇게 증가된 염소 이온 유입은 뉴런의 막 전위를 과분극시킨다. 결과적으로, 휴지 전위와 역치 전위 사이의 차이가 증가하고 활동 전위가 발생할 가능성이 줄어든다.

다른 GABAA 수용체 아형은 뇌의 다른 영역 내에서 다양한 분포를 가지며, 따라서 뚜렷한 생물학적 신경 회로를 조절한다. 따라서 벤조디아제핀에 의한 다양한 GABAA 수용체 아형의 활성화는 뚜렷한 약리학적 작용을 초래할 수 있다.[168]

벤조디아제핀이 결합하는 GABAA 수용체의 하위 집합은 벤조디아제핀 수용체(BzR)라고 한다. GABAA 수용체는 5개의 소단위체로 구성된 헤테로머이며, 가장 흔한 것은 두 개의 ''α'', 두 개의 ''β'', 하나의 ''γ''(α2β2γ1)이다. 각 소단위체에 대해 많은 아형(α1–6, β1–3 및 γ1–3)이 존재한다. 벤조디아제핀은 α1, α2, α3, α5 소단위체를 포함하는 GABAA 수용체에 결합한다.[171] 벤조디아제핀 수용체에 결합하면 벤조디아제핀 리간드는 벤조디아제핀 수용체를 GABA에 대한 친화력이 더 큰 형태로 고정시킨다. 이는 관련 염소 이온 채널의 개방 빈도를 증가시키고 관련 뉴런의 막을 과분극시킨다. 사용 가능한 GABA의 억제 효과가 증폭되어 진정 및 항불안 효과를 낸다. 예를 들어, α1에서 높은 활성을 갖는 리간드는 더 강한 수면제 효과와 관련이 있는 반면, α2 및/또는 α3 소단위체를 포함하는 GABAA 수용체에 대한 친화성이 더 높은 리간드는 우수한 항불안 활성을 갖는다.[172]

중추신경계에서는 신경전달물질로 아미노산이 많이 분포되어 있다. 주요 신경 작용성 아미노산으로는 흥분성 아미노산인 글루탐산, 억제성 아미노산인 GABA가 유명하다. 글루탐산 수용체 (나트륨, 칼슘 이온 채널)로는 이온 채널형 수용체인 AMPA 수용체, NMDA 수용체, 카이닌산 수용체가 잘 알려져 있으며, 대사성 글루탐산 수용체로는 mGluR이 알려져 있다. GABA 수용체에서는 이온 채널형인 GABAA 수용체 (클로라이드 이온 채널)와 G 단백질 연결 수용체인 GABAB 수용체가 알려져 있다.

GABAA 수용체에는 리간드인 GABA 결합 부위 외에 바르비투르산계 결합 부위, 벤조디아제핀 결합 부위, 당질 코르티코이드 결합 부위, 페니실린 결합 부위, 푸로세미드 결합 부위, 플루마제닐 결합 부위가 알려져 있으며, GABA와의 반응성을 조절하고 있다.

벤조디아제핀계 약물은 GABAA 수용체와 결합하면 채널의 개구 빈도를 증가시킨다. 바르비투르산계 약물과 달리 개구 시간을 연장하지 않고, 고용량 부하를 가해도 효능제 활성을 갖지 않는다. 그 점에서 바르비투르산계보다 안전성이 높다고 여겨져 항불안제, 수면제, 항전간제, 근이완제로 사용된다. 벤조디아제핀이 클로라이드 채널의 개구 빈도를 높이는 메커니즘으로는 GABA와 GABAA 수용체와의 결합 친화성을 높이기 때문이라고 생각된다.

GABAA 수용체의 주요 작용제는 바르비투르산계와 벤조디아제핀이 있지만, 바르비투르산계는 GABA의 최대 효력을 높이는 반면, 벤조디아제핀은 용량 효력을 높인다고 생각된다.

벤조디아제핀 수용체에는 3가지 하위 유형(중추성 ω1, ω2 및 말초성 ω3)이 알려져 있다. 대부분의 벤조디아제핀은 ω1, ω2를 구별하지 않는다. ω1은 진정에 관여하고, ω2는 인지, 기억, 운동 기능에 관여한다고 생각되지만, 아직 명확히 밝혀지지 않았다.

7. 약물 상호 작용

개별 벤조디아제핀은 특정 약물과의 다양한 약물 상호작용을 가질 수 있다. 대사 경로에 따라 벤조디아제핀은 대략 두 그룹으로 나눌 수 있다. 가장 큰 그룹은 시토크롬 P450(CYP450) 효소에 의해 대사되며 다른 약물과의 상호 작용 가능성이 상당한 그룹이다. 다른 그룹은 글루쿠론산화를 통해 대사되는 로라제팜, 옥사제팜, 테마제팜과 같이 일반적으로 약물 상호 작용이 거의 없다.[164]

경구 피임약, 일부 항생제, 항우울제, 항진균제를 포함한 많은 약물은 간에서 시토크롬 효소를 억제한다. 이는 CYP450에 의해 대사되는 벤조디아제핀의 제거 속도를 감소시켜 약물 축적이 과도해지고 부작용이 증가할 수 있다. 반대로, 세인트 존스 워트, 항생제 리팜피신, 항경련제 카르바마제핀 및 페니토인과 같은 시토크롬 P450 효소를 유도하는 약물은 많은 벤조디아제핀의 제거를 가속화하고 작용을 감소시킨다.[166][165] 벤조디아제핀을 알코올, 오피오이드 및 기타 중추 신경계 억제제와 함께 복용하면 작용이 증폭된다. 이는 종종 진정 효과 증가, 운동 조절 장애, 호흡 억제 및 치명적인 결과를 초래할 수 있는 기타 부작용으로 이어진다.[166][165] 제산제는 일부 벤조디아제핀의 흡수를 늦출 수 있지만, 이러한 효과는 미미하고 일관성이 없다.[166]

8. 대한민국 현황

대한민국에서 벤조디아제핀은 마약류 관리에 관한 법률에 따라 향정신성의약품으로 지정되어 엄격하게 관리되고 있다.[349] 벤조디아제핀은 오남용 우려 의약품으로 지정되어 처방 및 조제 시 주의가 필요하다. 대한신경정신의학회는 벤조디아제핀 장기 처방 가이드라인을 통해 안전한 사용을 권고하고 있다.
